About this file

This document is a First Article Test (FAT) Waiver Worksheet template used by contractors to request waivers from mandatory first article testing requirements in federal contracting. The worksheet is designed to be submitted on company letterhead with an authorized agent's signature and must be attached to Section J of the solicitation.

The worksheet requires contractors to provide current acquisition information including the contracting officer's name, solicitation/contract number, contractor name and CAGE code, item nomenclature, part number, NSN, and top drawing number. Contractors must specify whether they are requesting a full waiver (all FAT requirements waived) or partial waiver (specific tests only), with reference to the applicable Technical Data Package (TDP) specifications. The justification section allows contractors to select from four primary basis categories: (A) successful completion of FAT on a recent contract for the same item using identical facilities, equipment, processes, and subcontractors; (B) successful FAT on a similar item tested to the same requirements; (C) material in continuous production with documentation of last successful FAT approval; or (D) successful FAT on a next higher assembly to eliminate duplication. Contractors may also claim other grounds under category (E) with specific justification. Additional sections require disclosure of quality issues (PQDRs, corrective actions), manufacturing data changes, and Material Review Board actions. Supporting documentation must be attached, and the submitter must certify the accuracy of all statements with printed name, title, company address, phone number, and signature.
