About this file

Form B is a Business Enterprise Good Faith Efforts explanation document issued by the City of St. Petersburg, Florida, for use by prime contractors on the Citywide Custom Uniform & Apparel Services procurement (IFB 26-208). Prime contractors who have not used any certified Business Enterprise subcontractors or suppliers, or who have failed to meet the City's established Business Enterprise participation goals, must complete this form to document their good faith efforts to secure Business Enterprise participation. The form requires contractors to demonstrate efforts such as conducting at least one monthly outreach event, documenting all solicited responses and quotes from Business Enterprise firms, maintaining a list of contacted but unavailable Business Enterprises with supporting documentation, attending City-sponsored outreach and networking events, and effectively utilizing the services of the Office of Supplier Development and other available small business organizations. Contractors must provide a detailed written explanation of all actions taken to meet Business Enterprise participation requirements, supported by comprehensive documentation of their recruitment and placement efforts. The form requires certification under penalty of perjury, with false statements subject to federal or state prosecution.

This procurement seeks qualified vendors to provide customized City apparel and related services under a multi-year agreement with an initial three-year term and two optional one-year renewal periods. The market basket includes 43 line items covering various apparel categories and customization services, with estimated annual usage quantities ranging from 30 to 2,400 units. Vendors must meet minimum qualifications including three years' experience, at least two government references, ability to complete alterations at no additional cost, and delivery within 15 business days. The response deadline is May 14, 2026, at 3:00 PM, with award based on the lowest responsive and responsible bidder evaluation. A 5 percent bid bond is required at submission, and upon award, the selected vendor must provide a performance bond equal to 100 percent of the bid amount and maintain commercial general liability insurance of at least $1,000,000 per occurrence and $2,000,000 aggregate. Pricing is firm for the first twelve months, with potential adjustments thereafter requiring written justification and mutual agreement from both parties.
