About this file

This document is a Past Performance Questionnaire used by the 412th Civil Engineering Group at Edwards Air Force Base (EAFB), California, to evaluate contractor performance as part of a competitive source selection process for a Single-Award, Indefinite-Delivery/Indefinite-Quantity (IDIQ) contract for multi-discipline Architect-Engineer (A-E) services. The questionnaire is designated as source selection sensitive information under FAR 2.101 and 3.104.

The questionnaire collects detailed information from organizations with direct experience with offerors, including contractor name, address, point of contact, project title, contract number, dollar amount, performance period, and the contractor's role (prime, sub, or key personnel). Respondents must rate contractor performance across 23 evaluation criteria using a six-point scale: Exceptional (E), Good (G), Satisfactory (S), Marginal (M), Unsatisfactory (U), and Neutral (N). The criteria encompass management capabilities, personnel qualifications, quality control procedures, schedule performance, problem resolution, responsiveness to changes, cost proposal accuracy, and overall contractor cooperation. The questionnaire requires narrative explanations for any Marginal or Unsatisfactory ratings and includes yes/no questions regarding cure or show cause notices, contract terminations (default, convenience, or pending), future award recommendations, and warranty defect resolution. Completed questionnaires must be submitted electronically to Adam R. Confer (adam.confer.2@us.af.mil) and Lucas Jochem (lucas.jochem@us.af.mil), with instructions to include separate forms for any other contracts held with the agency within the past three years.

One of the considerations in proposal evaluation is the verification of the offerors' past performance on contracts or other work efforts, which reflect the offeror's ability to perform on the proposed effort. Our areas of interest regarding the offeror are summarized in the enclosed questionnaire.

Our schedule is extremely tight and we desire your written response to be submitted along with your proposal package.

To assist you in preparing your response and expediting your reply, the questionnaire may be filled out electronically and e-mailed to adam.confer.2@us.af.mil. Please call Adam R. Confer at (661) 277-3932 if you have any questions. Your completed questionnaire will become a part of the official Source Selection records.

Your assistance is greatly appreciated and your prompt response will be one of the keys to the successful and timely completion of this Source Selection.
