This Operating Instruction (OI) implements Air Force Policy Directive (AFPD) 32-70, Environmental Quality and AFI 32-7086, Hazardous Materials Management. This OI DOES NOT apply to the Aerospace Systems Directorate located at Wright Patterson Air Force Base, OH. The purpose of this OI is to provide environmental, security and safety guidelines, as well as approval, purchasing delivering and receipt requirements for Hazardous Material (HM) acquisition at AFRL/RQ-W, Edwards AFB. This OI applies to all government and contractor personnel working at AFRL/RQ-W. Refer recommended changes and questions about this publication to the Office of Primary Responsibility (OPR) using the AF IMT 847, Recommendation for Change of Publications; route AF IMT 847s from the field through the appropriate functional chain of command. Ensure that all records created as a result of processes prescribed in this publication are maintained in accordance with AFMAN 33-363, Management of Records, and disposed of in accordance with the Air Force Records Disposition Schedule (RDS) located at https://afrims.amc.af.mil/. 1.0. INTRODUCTION:
1.1. This OI identifies the process for approving, ordering, delivering and issuing HM at AFRL/RQ-W.
1.1.1. Do not use this OI for approvals/shipments of Hazard Class 1 explosives (see RQ-W 21-201, Munitions Customer Guide for explosives management)
1.2. Personnel may not acquire, receipt or use a HM at AFRL/RQ-W until they receive all required authorizations via the processes described in this OI.
There are two methods to obtain a HM authorization:
1.2.1. For standard industrial HM requests, authorizations are submitted and approved through the Enterprise Environmental Safety and Occupational Health Management Inventory System (EESOH-MIS). Requests are submitted to the Hazardous Material Pharmacy (HAZMART) for input into EESOH-MIS.
HM approved through EESOH-MIS will be tracked in EESOH-MIS. (See para
2.1.3 for authorization process).
1.2.2. For research chemicals, authorizations are submitted and approved through the AF Form 3952, Chemical/Hazardous Material Request Authorization Form program located in the main directory of the ESOH database. HM approved through the AF Form 3952, Chemical/Hazardous Material Request Authorization Form database will be input and tracked by the owner/user in the web-based Chemical Inventory System (CISPro) upon receipt. (See para 2.1.4 for authorization process).
1.3. With the exception of bulk HM, all incoming HM (industrial or research) will be delivered to the Logistics Material Control Activity (LMCA) Hazardous Material Temporary Storage area (HAZ-CELL), Building 8460.
1.3.1. Personnel who receive bulk HM shipments (gas cylinders, tanker trucks/tube trailers, etc) will immediately contact the HAZMART (5-5592) to report all deliveries. Do not deliver bulk HM to the HAZ-CELL.
1.4. Personnel will not transport HM in a Privately Owned Vehicle (POV) under any circumstance, either on or off base. Department of Defense (DoD) or contractor rented/lease vehicles may be used to transport HM.
1.5. At no time will government purchased HM or explosives be transported to or stored at a private residence.
1.6. HM not reported/processed as required will be impounded until all processing actions are completed. HM authorization may be suspended if discrepancies /issues persist.

2.0. PROCEDURES:
2.1. OBTAINING A HAZARDOUS MATERIAL AUTHORIZATION:
2.1.1. The HAZMART (5-5592) is the customer focal point for all HM processing and support.
2.1.2. All HM approval requests submitted via EESOH-MIS or the AF Form 3952, Chemical/Hazardous Material Request Authorization Form database are coordinated and approved by the Environmental, Safety and Bioenvironmental offices.
2.1.3. EESOH-MIS Authorizations (non-research chemicals):
2.1.3.1. HM requester will consult with the HAZMART to
determine if an item is currently loaded under an approved process. If item is loaded, proceed to para 2.2.
2.1.3.2. Submit the following to the HAZMART
(afrl.pharmacy@edwards.af.mil):
2.1.3.2.1. HM name.
2.1.3.2.2. Process (how HM will be used).
2.1.3.2.3. Amount of HM used per process.
2.1.3.2.4. Number of processes per day, month
or year.
2.1.3.2.5. Duration of each process (minutes, hours, days, etc).
2.1.3.3. When a HM request is approved, the HAZMART
will notify the requester and the HM may now be ordered (para 2.2).
2.1.4. AF Form 3952, Chemical/Hazardous Material Request Authorization Form ESOH Data Base Authorizations (research chemicals):
2.1.4.1. HM requester will check the 3952 ESOH Data Base
(Hazmat List) or consult with the HAZMART to determine if an item is currently approved and loaded in the database. If loaded, proceed to para 2.2.

2.1.4.2. In the 3952 ESOH Data Base, complete and submit
a ‘Hazmat Request & Authorization’ form under ‘New License Request’. Click ‘Notify Certifier’ to start processing.
2.1.4.3. When a HM request is approved, the HAZMART
will notify the requester via e-mail the HM may now be ordered.
2.1.4.4. The requester will review the HM authorization for all restrictions that may apply and provide the organization’s purchaser with the HM approval number.
2.1.4.5. Once an order has been submitted by purchaser, the
requester will return to the 3952 database, ‘Hazmat Request & Authorization’ and click on the “order material”. This process will notify LMCA and the HAZMART that HM order has been placed. It will also provide LMCA with the POC for the HM and the Material Safety Data Sheet (MSDS).
2.2. HAZARDOUS MATERIAL PURCHASE REQUEST:
2.2.1. Contractor purchase orders must be validated through the HAZMART (phone: 5-5592) prior to purchasing HM.
2.2.1.1. HM ordered through eTracks must have the authorization number (3952 orders only) in the comments section. eTracks orders received without an authorization number will be returned to requester.
2.2.1.2. Hard Copy Purchase requests must have signed approval from the HAZMART.
2.2.2. When using the Government-wide Purchase Card (GPC), ensure HM purchase and delivery confirmations are emailed to LMCA and the HAZMART at the following addresses: afrl.lmca@edwards.af.mil, afrl.pharmacy@edwards.af.mil). This ensures compliance with AFI 64-117, Air Force Government-wide Purchase Card Program, for GPC cardholders.
The confirmations must have the following information:
2.2.2.1. The estimated delivery date.
2.2.2.2. Name, branch and duty phone of all personnel who may receipt the item.
2.2.3. The purchaser will ensure the following information is provided to the vendor when placing a HM order:

2.2.3.1. RQ-W delivery point is 302 North Mercury Boulevard, Edwards AFB 93524, Bldg 8460 (HAZ-CELL), unless a bulk shipment or an alternate location has been pre-approved by the HAZMART.
2.2.3.2. Delivery hours are 0730-1530.
2.2.4. The purchaser must ensure the following when placing HM order:
2.2.4.1. If the HM manufacturer is not the same as that reflected on the HM authorization, the HM cannot be ordered unless a current MSDS is provided and the change was approved by the HAZMART.
2.2.4.2. If the HM order is delayed or canceled, the requestor will notify LMCA and the HAZMART.
2.3. MATERIAL HANDLING UNIT (DELIVERY AND PROCESSING):
2.3.1. The Material Handling Unit (MHU) will maintain copies of all processed requests and related messages on file until the shipment is complete.
2.3.2. Upon delivery, the MHU will validate that the HM received is the same as that ordered.
2.3.2.1. If there are any discrepancies between the purchase
request and the HM being delivered, the MHU will notify the HAZMART. The HAZMART will request the purchaser determine the cause of the discrepancy, correct the EESOH-MIS/3952 database to reflect the correct HM information, and ensure the discrepancy is not repeated for future orders.
2.3.3. The MHU will move the HM to the HAZ-CELL.
2.3.4. The MHU will ensure the HAZMART and the HM requesting
organization are contacted and advised that HM is awaiting receipt at the HAZ-
CELL.
2.3.5. The HAZMART will contact the MHU by 1530 to ensure all HM has been collected and is no longer at the HAZ-CELL. Any remaining HM will be collected by the HAZMART and stored at their location until the requesting organization collects the HM.

2.4. RECEIPT OF HM FROM HAZ-CELL (FACILITY 8460):
2.4.1. Privately Owned Vehicles are not authorized to transport HM.
2.4.2. Upon notification, the requesting organization will immediately send a representative to the HAZ-CELL to sign for and receive HM.
2.4.3. If the HM is not picked up by 1530 hours, HM will be collected by the HAZMART and stored at their location until the requesting organization collects the HM.
2.4.3.1. The HAZMART will notify the requesting organization
that they must pick up HM from the HAZMART (Facility 8425).
2.4.3.2. HM not receipted within 48 hours may be returned
to the vendor at requestors cost or deemed waste and processed for disposal.
2.4.4. Receipted HM must be entered into the EESOH-MIS or CISPro tracking systems within 48 hours of receipt. Failure to properly receipt and track HM may result in material being removed from organization and disposed of.
