About this file

This document is a past performance questionnaire for a federal solicitation. The Air Force Civil Engineering Center Installations Directorate is seeking advisory and assistance services for managing its housing, enhanced use lease, and utility privatization programs on a long-term basis across the United States. Responses to the questionnaire are due within 10 calendar days and will be used to evaluate offerors' past performance on housing and utility privatization contracts. The questionnaire collects information on technical performance, program management, transition and phase-in, employee retention, small business participation, and cost performance for relevant prior contracts. Respondents are asked to rate performance in these areas on a scale of exceptional to unsatisfactory and provide supporting details. The completed questionnaires will become part of the official source selection records for solicitation FA8903-20-R-0003, which seeks a contractor to provide housing, enhanced use lease, and utilities privatization management services.

Section L: Present/Past Performance Questionnaire

For Official Use Only Page 1 of 8

The Air Force Civil Engineering Center Installations Directorate (AFCEC/CI) of the Air Force Material Command (AFMC) is in the process of competitively selecting a source(s) for Housing, Enhanced Use Lease and Utility Privatization (HEU) Management program. HEU provides for long-term cradle to grave management of Advisory & Assistance Services (A&AS) in support of the AF privatized programs. This includes Asset, Acquisition and Portfolio Management services in support of HEU throughout the United States.

One of the considerations in proposal evaluation is the verification of the Offerors' past performance for Housing and Utility Privatization contracts or other work efforts which reflect the Offeror's ability to perform on the proposed effort. We depend on information received from organizations such as yours, which have had first-hand experience with the Offeror, for the evaluation of the Offeror's performance on those contracts or work efforts.

Our areas of interest regarding the Offeror are summarized in the enclosed questionnaire. In order to meet the acquisition milestones, we request your written response no later than 10 calendar days after your receipt of this letter. This schedule will allow us sufficient time to analyze the data prior to the start of negotiations.

To assist you in preparing your response and expediting your reply, submit the questionnaire electronically and e-mail to Michael Grasso, email: michael.grasso.3@us.af.mil and Linda Cavazos, email: linda.cavazos@us.af.mil.

Please call or email Linda L. Cavazos, email: linda.cavazos@us.af.mil, Contracting Officer at M: 210-395-8730/C;

210-347-9488, if you have any questions. Title of email: RFP FA8903-20-R-0003, Present/Past Performance Questionnaire. Your completed questionnaire will become a part of the official Source Selection records.

Your assistance is greatly appreciated and your prompt response will be one of the keys to the successful and timely completion of this Source Selection.
