Please indicate your satisfaction with the contractor’s performance by placing an “X” in the appropriate block using the scale provided to the right of each question. This scale is defined as follows:

CODEPERFORMANCE LEVEL
BBLUE/EXCEPTIONAL - The contractor’s performance meets contractual requirements and exceeds many (requirements) to the Government’s benefit. The contractual performance was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.
PPURPLE/VERY GOOD- The contractor’s performance meets contractual requirements and exceeds some (requirements) to the Government’s benefit. The contractual performance was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
GGREEN/SATISFACTORY – The contractor’s performance meets contractual requirements. The contractual performance contained some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
YYELLOW/MARGINAL – Performance does not meet some contractual requirements. The contractual performance reflects a serious problem for which the contractor has not yet identified corrective actions or the contractor’s proposed actions appear only marginally effective or were not fully implemented.
RRED/UNSATISFACTORY – Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance contains serious problem(s) for which the contractor’s corrective actions appear or were ineffective.

a. Systems Engineering: Utilized a systems engineering process that applied emerging systems engineering initiatives and practices. Utilized engineering methods to reduce risk of obsolescence.

b. Configuration Management (CM) and Control: Utilized a CM process to implement and maintain CM and change control throughout the training system/device life cycle, to include management of multiple/overlapping baselines when necessary.

c. Concurrency Modifications: Demonstrated a change management process for maintaining training system concurrency.

a. Training System Support Center (TSSC): Effectively performed and/or managed TSSC operations, responsibilities, and interaction with development teams.

b. Contractor Logistics Support (CLS): Effectively managed support resources, technical data, spares, support equipment, maintenance, Government-Furnished Property (GFP) accountability, device status reporting, student throughput, and technicians to sustain training systems and ensure normal and surge trainer availability requirements.

· Rapid repair to maintain training system availability in sustainment.

c. Distributed Mission Operations (DMO): Effectively executed DMO requirements to include resource allocation, scheduling, coordination, establishing connectivity for pre-mission briefings, mission execution, post-mission debriefing activities, and security.

d. Diminishing Manufacturing Sources and Materiel Shortages (DMS/MS): Effectively forecasted, identified, monitored, and mitigated deficiencies due to DMS/MS and obsolescence issues within the training system.

e. Efforts to obtain, hard-to-find, or one-of-a-kind-spares, as part of the obsolescence effort.
