About this file

This is a Past Performance Questionnaire template for solicitation FA524026Q0034 issued by the 36th Contracting Squadron in Guam. The questionnaire is designed to collect past performance information from contractors bidding on Basewide Utility Vehicle Maintenance and Repair Services at Andersen Air Force Base (AAFB) in Guam. Prospective contractors must submit completed questionnaires to elena.gifford.1@us.af.mil and lauren_renee.reyes.2@us.af.mil no later than 10 September 2026 at 10:00 AM Chamorro Standard Time (ChST).

The questionnaire requires contractors to provide general information including company name, address, point of contact details, project title and description, contract number, overall contract value, and contract performance dates, along with identification of whether the contractor performed as a prime contractor or subcontractor. Additionally, the questionnaire mandates evaluation across six performance criteria—Quality of Service, Schedule, Cost Control, Management of Key Personnel, Small Business Subcontracting, and Regulatory Compliance—each rated on a five-point adjectival scale (Exceptional, Very Good, Satisfactory, Marginal, or Unsatisfactory). For any Marginal or Unsatisfactory ratings, contractors must provide substantiating comments. The questionnaire is a component of the broader solicitation for commercial utility vehicle maintenance and repair services, which is set-aside for small business concerns.
