About this file

This document is a Past Performance Questionnaire (PPQ) to be completed by the government customer for a federal contract opportunity. The PPQ is part of the source selection process for the Multiple Award Construction Contract (MACC) program at Moody Air Force Base, Georgia. The MACC is an indefinite-delivery, indefinite-quantity contract that will provide a broad range of design-build, maintenance, alteration, repair, renovation, and minor construction services on real property. The government plans to award 9 MACC contracts, including 3 each for 8(a), HUBZone, and small business categories. The PPQ requests information on the contractor's past performance in areas such as project management, technical performance, safety/labor compliance, customer satisfaction, change/cost control, and quality control. The completed PPQs are to be submitted directly to the government points of contact and are considered source selection information.
