Network Access Request Procedures
1. Apply for a CAC card through your COR/Rep. (Do not proceed until you have a CAC)
2. Check in with Security to complete required forms and in process. Tracy Ebron 878-2081 or Larry Ondic 878-0328.
3. Create an AKO account at: https://amid.us.army.mil/suite/page/eamsarequests Depending on the user circumstances, you may need an AKO sponsor. If this is your first time requesting an AKO account or have retired from the military you qualify for a self-sponsored account without sponsorship.
If sponsorship is required for your account, below are the sponsors:
Beverly.F.Hosey => Supply, Fuels and LRD OPS robert.s.mcnaughton => Transportation including PPO william.h.pettaway => TMP and Rail
4. Army Training Certification & Tracking System (ATCTS): If you were prior military or have an ATCTS account, please let the IMO know. They will pull your account in by your email or DOD ID number (EDIPI). If not, the IMO will register you into ATCTS.
5. Complete your DOD Cyber Awareness Challenge training at (https://cs.signal.army.mil/login.asp) Do not complete this training until you have a CAC and ATCTS profile created.
6. Complete the Acceptable Use Policy (AUP) form from the site in step 5 above.
7. Complete the DD Form 2875. (Note: Signature section requires a digital signature) A. Type of Request: Select “INITIAL” and enter your EDIPI (10 digit number on the back of you CAC card) next to “USER ID”.
B. Complete the “Date” field.
C. Complete the “SYSTEM NAME” field (enter: NAE Network Account)
D. Complete the “LOCATION” field (enter: Fort Eustis, VA)
E. Complete PART I:
a. Name
b. Organization example: 733rd MSG /(Contract Name)or (LRD)
c. Office Symbol: Leave Blank if unknown
d. Phone
e. Official E-Mail: Enter your Enterprise Email address (the user will show in the Address Book under “Global Address List”. The Alias will be their Enterprise Email Account with the @mail.mil added)
f. Job Title and Grade:
g. Official Mailing Address: Your work address
h. Citizenship
i. Designation of Person
j. IA Training: Enter the date you completed your IA training.
k. Digitally sign Part I
F. Complete PART II 13 (enter justification for access) G. Complete # 14 select (Authorized)
H. Forward to your Supervisor if Civilian to complete blocks 16-20b. If you’re a Contractor forward to your COR to complete blocks 16-20b
I. IMO complete blocks 22-25
J. Security Officer Complete Blocks 28-32
8. Send the request via email to Jose.lopez3.civ@mail.mil and Haneefa.r.brogdon.civ@mail.mil with DD Form 2875 attached.
9. Once the IMO have verified your cyber security training in ATCTS, received your DD Form 2875 and verified AUP, they will create your network account.
10. The user will update their GAL information via DMDC before entering a request for Email Provisioning.
11. The user will call the AESD helpdesk at (878-4071) to request their Enterprise Email account to be provisioned. Request a Business Enterprise Email Account.
12. Your email account should be available in 24 hours after request is submitted.
