1. General Information

1.1. Scope: The contractor shall provide all travel, personnel, equipment, tools, materials, parts, transportation, supervision, and quality control necessary to perform elevator inspections, preventive maintenance, and service calls, as defined in this Performance Work Statement

(PWS).

1.2. Background: This full maintenance contract signifies an elevator service provider (Contractor) takes total service responsibility for the equipment identified in the contract. The monthly fixed price includes all inspections, maintenance, repairs (labor) under two hours, and parts under $500.00, for both routine and emergency service calls. This contract allows the government to budget total yearly annual costs and eliminates concerns relating to individual parts repair or replacement invoicing. Service specific requirements are identified in paragraph 3 Requirements.

1.3. Anticipated Period of Performance (PoP): The Period of Performance shall be one base year of 12 months and four 12-month option years for a total period of five years. A onetime extension of 6 months at the end of this period of performance may be determined to be in the best interest of the Government, see FAR 52.217-8. The award document will call out the PoP and is the definitive performance period.

1.4. Recognized Holidays: Mission essential personnel are required to work on holidays if necessary. This contract has not been deemed mission essential. The following are recognized United States (US) holidays. If a holiday falls on a weekend, the Government will have an observed day off on the preceding Friday or following Monday. The contractor shall perform emergency services on these days if specifically directed. 1.5. Place and Performance of Services. The contractor shall provide routine services between the hours of 0700-1600 Monday through Friday except on recognized US holidays or when the Government facility or Whiteman AFB is closed due to local or national emergencies, administrative closings, or similar Government-directed facility or Whiteman AFB closings.

Emergency services shall be 24 hours per day 7 days a week. Performance shall be at Whiteman AFB, Missouri, at the facilities listed in Appendix A. The contractor at all times shall maintain an adequate work force for the uninterrupted performance of all tasks defined within this PWS.

When hiring personnel, the contractor shall keep in mind that the stability and continuity of the work force are essential.

1.6. Schedule Coordination: The Contractor shall be responsible for coordinating all phases of his/her operations with the appropriate base personnel through the CO/COR. The facilities shall remain in operation while the Contractor is working and it is up to the Contractor to coordinate around the normal activities of the facility.

1.7. Special Required Qualifications: The Contractor shall perform necessary inspections and tests as required under OSHA 1917.116 and ASME A17.1 Appendix N Table N-1-1, using a Qualified Elevator Inspector (QEI). The Contractor will be licensed by the state in which the work is conducted to provide the services specified in this contract. All work will be performed by personnel who are trained and qualified for the systems and equipment. Contractor personnel shall be certified by appropriate federal and state regulatory agencies to meet federal and local certification requirements (documentation provided to CO/COR upon request) in maintenance of VTE. A copy of certificate/license for each technician shall be provided to the CO/CO within 10 days of contract award and prior to commencement of work. In the instance where a new or replacement technician is hired, a copy of certificate/license will be provided within 10 days of hire and prior to commencement of work by that employee.

1.8. Service Contract Act: This contract is subject to the most recent Service Contract Act.

The most recent stated wage rate for an elevator repairer as of 12 Feb 2025, occupation code 23210 is $60.69/hour.

1.9. Identification of Contractor Employees: All contractor personnel attending meetings and working in situations where their contractor status is not obvious to third parties are required to identify themselves as such to avoid creating an impression that they are Government employees.

The contractor shall ensure that all documents or reports produced by contractor personnel are suitably marked as contractor products or that contractor participation is appropriately disclosed.

The contractor’s status as a “contractor” shall be predominantly displayed in all correspondence types (to include signature blocks on e-mail) and dealings with Government or non-Government entities. All contractor personnel shall wear a badge or logoed clothing displaying the company name while on the base. The badge or logo shall be evident to the casual observer.

1.10. Government Provided Identification: All contractor personnel accessing the base will be required to obtain a base access badge (provided by Government) and that badge shall be worn and clearly displayed at all times while on the base. Contractor personnel requiring access on base must be U.S. citizens in order to obtain a base access badge. Non-U.S. citizens are not authorized unescorted entry on Whiteman AFB and therefore cannot fulfill this contract. The contractor shall return all identification media (i.e., badges and vehicles passes) to the Government within seven days of an employee’s departure or upon immediate request of the Government if identification is required for particular one-time or case-by-case access.

1.11. Environmental Control. The Contractor shall comply, and assure that all subcontractors comply, with all applicable federal, state, and local laws, regulations, ordinances, policies, and standards related to environmental matters. Where applicable, the Contractor shall use environmentally safe products in the course of completion of their project. The Contractor shall maintain, in company vehicle on site, Safety Data Sheets (SDS) for all chemicals. A copy of all SDS will be provided to the CO/COR. The SDS data is subject to random checks by the government. Contractor shall maintain, and provide on demand, an inventory of materials being

P a g e 4 | 21 brought on the Government facility. The Contractor shall complete, and provide on demand, monthly inventories of HAZMAT used, including but not limited to: solvents, paints, degreasers, greases, Ozone Depleting Substances, and oils brought on to the Government facility. If the Contractor spills or releases any HAZMAT or other substance contained in 40 CFR 302 into the environment, the Contractor shall immediately notify the CO/COR and appropriate emergency responders. The Contractor is responsible for all costs associated with clean up and restoration, including any applicable fines and/or penalties. The Contractor shall maintain a spill plan as required by federal, state, and local laws and regulations. (The bases should coordinate this requirement with their base environmental office and edit as required to comply with local environmental policies.)

1.12. Confined Spaces: Within 30 days of contract award, the Contractor shall develop and submit a comprehensive written Confined Space Entry Program. This program shall be compliant with all applicable federal, state, and local regulations, and shall address, at minimum, the following elements:

• Hazard Identification and Assessment: Procedures for identifying and evaluating potential hazards associated with each confined space.

• Control Measures: Engineering controls, administrative controls, and personal protective equipment (PPE) required to mitigate identified hazards.

• Atmospheric Monitoring: Procedures for testing and monitoring the atmosphere within confined spaces before and during entry.

• Rescue Procedures: Plans and procedures for the safe rescue of authorized entrants in case of emergency.

• Training and Authorization: Requirements for training and authorizing personnel for confined space entry, including specific roles and responsibilities.

• Program Review and Updates: A process for periodically reviewing and updating the program to ensure its effectiveness and compliance.

1.13. Asbestos: The Contractor shall comply with Section 29 CFR 1910.1001(j)(7)(iv). VTE may be located in facilities that may contain ACM or PACM. The Contractor (not the Government) shall determine if any exposure to ACM may result in performing work under the contract. See Asbestos Report (Solicitation Attachment 4).

2. Whiteman AFB Information

2.1. Unscheduled Gate Closures: As determined by the Security Police, gate closures may occur at any time causing all personnel entering or exiting a closed installation to experience a

P a g e 5 | 21 delay. This cannot be predicted or prevented. Contractors are not compensated for unexpected closures or delays. Vehicles operated by contractor personnel are subject to search pursuant to applicable regulations. Any moving violation of any applicable motor vehicle regulation may result in the termination of the contractor employee’s installation driving privileges. Scheduled work not accomplished because of base closure due to weather, exercise, or actual alert will be accomplished as soon as possible after reopening of the base. Occasionally, it is necessary to require early release of Whiteman AFB personnel, late reporting, or a total base closure when weather conditions make normal travel unsafe. When weather conditions cause one of the above situations, Contractor personnel will be denied access to the base, unless they have been notified their contract service has been determined essential. Occasionally, real world situations or exercises may prohibit base access or ability to remain on base. 2.2. Installation Regulations: The contractor’s employees shall become familiar with and obey the regulations of the installation; including fire, traffic, safety, and security regulations while on the installation. Contractor employees should only enter restricted areas when required to do so and only upon prior approval. All contractor employees shall carry proper identification with them at all times and shall be subject to such checks as may be deemed necessary. The contractor shall ensure compliance with all regulations and orders of the installation that may affect performance. The Government reserves the right to direct the removal of an employee for misconduct, safety, or security reasons. Removal of contractor employees for reasons stated above does not relieve the Contractor from responsibility for total performance of this contract.

2.3. Base Access: All personnel entering Whiteman AFB must possess a valid State or Government picture identification card. As of 7 May 2025, Whiteman AFB requires that visitors have a REAL ID to enter base. Individuals presenting identification cards from a state that is non-compliant with the REAL ID Act will require additional documentation to gain unescorted base access. The only alternative forms of ID for visitors to use in conjunction with their non-compliant REAL ID credential include: birth certificate with a raised seal and social security card, valid U.S. or foreign Passport/Passport Card, Veteran’s Health Identification Card, or Transportation Worker Identification Card. The contractor shall comply with all applicable Whitman AFB or facility access along with local security policies and procedures, which may be obtained from the COR or other Government contact. The contractor and all associated subcontractor employees shall provide all information required for background checks to meet installation access requirements to be accomplished by installation Security Forces. The contractor shall ensure compliance with all personal identity verification requirements as directed by Department of Defense (DoD), Headquarters Department of the Air Force, and local policy. Should the Force Protection Condition (FPCON) change, the Government may require changes in contractor security matters or processes. P a g e 6 | 21 obtained during identity vetting that indicates the individual may present a threat to the good order, discipline, and morale of the installation. 2.3.1. Background Investigation Requirements: This contract requires a Missouri Uniform Law Enforcement System (MULES) verification of all contractor employees or subcontractor employees to obtain a base access badge. The MULES verification and associated documentation may take a week or longer to process. The contractor shall make all reasonable efforts to ensure contractor employees meet MULES verification standards upon assignment to the contract. The contractor shall be held responsible for delays, failure to meet performance requirements or decreases in efficiency in accordance with the applicable inspection clause.

2.4. Physical Security: The contractor shall safeguard all Government property provided for contractor use. At the close of each work period, Government facilities, equipment, materials shall be secured, and building personnel notified.

2.5. Lock Combinations: The contractor will be given access to locked sites normally through keys. The contractor shall establish and implement methods of ensuring the security of the key and not given to unauthorized persons. The contractor shall ensure if a key was issued it is returned to the appropriate personnel before departing that building.

2.6. Facility Clearance: Certain facilities will require the contractor to be escorted at all times while in that facility. The escort will be provided by the government.

3. Requirements

The contractor shall provide all personnel, travel, equipment, tools, materials, supervision, and quality control necessary to maintain, repair, and inspect the real property vertical transportation equipment (VTE) at Whiteman AFB MO in a manner that will ensure continuous and safe operation. This full maintenance contract signifies an elevator service provider (Contractor) takes total service responsibility for the equipment identified in the contract. The monthly fixed price includes all inspections, maintenance, troubleshooting, repairs, replacements, and routine and emergency service calls. The monthly fixed price includes all inspections, maintenance, repairs (labor) under two hours, and parts under $500.00, for both routine and emergency service calls. This contract allows the government to budget total yearly annual costs and eliminates concerns relating to individual parts repair or replacement invoicing. Elevator inspections shall be in compliance with the most current 11 Code of Regulations (CSR) 40-5. The current 11 CSR 40-5 is dated 31 March 2020. Work conducted on elevators shall be in accordance with

(OSHA) Standards and the recommendations of the original equipment manufacturers

(OEM).

General Description: The following description shall serve as general information along with performance objectives as required by the Government. Unmentioned tasks shall not limit the contractor’s obligation to conform to all state and local regulations (to include safety and environmental) in accordance with standard industry and commercial practice. Troubleshooting, preventive maintenance, and repairs shall be on all elevators identified in attachment and are included in the scope of services. All troubleshooting, preventive maintenance, repairs, and replacement of damaged, broken, or worn parts are actions to restore equipment to a state that in which the equipment may be operated safely and in accordance with the manufacturer’s recommendations. See ASME 17.1 for requirements.

3.1 Task 1. Preventative Maintenance and Inspections Services

3.1.1 The Contractor with the Contracting Officer (CO)/Contracting Officer’s Representative (COR) shall determine the working order and condition of all elevators listed in Appendix A within the thirty calendar days of contract award. The Government will either replace missing items or repair all items not in working order or serviceable condition or direct the Contractor to replace the missing items, accomplish the repair, and reimburse the Contractor in accordance with the contract. The CO will give instructions for situations where a unit requires a level of repair potentially constituting alteration. The Contractor and the CO/COR will certify their agreement as to the working order of the equipment. All repair work shall be in accordance with standard commercial practices using only new parts of equal quality as specified by the elevator’s manufacturer. Substitution of a different component is only permitted where it is equivalent to that which is tested, as determined by the certifying agency for that part or component.

3.1.2 The contractor shall perform preventative maintenance monthly on all elevators listed in Appendix A. The preventative maintenance shall include as a minimum, but not limited to fire service testing, two-way communication, emergency lighting and bell, Machine room equipment, pump, elevator shaft, elevator interior, dirt and debris accumulation, moisture accumulation, and any obvious defects that can affect the operability of the elevator.

3.1.3 The contractor shall perform inspections and preventive maintenance every six months on all elevators listed in Appendix A. Posting is required of only the annual certificate of inspection IAW 11 CSR 40-5. All elevator inspections will be performed in the same month(s) each year. The contractor shall provide inspection reports on all elevators within two business days following the inspection to both the COR and CO/CA. The initial inspections and preventive maintenance shall commence the month the contract is awarded and the second performance for the contract year will be six months later. That schedule will be maintained for the duration of the contract.

3.1.4 The Contractor shall develop and submit to the government an annual written Maintenance Control Program (MCP) for each elevator within thirty days after contract award to cover the basic contract. An updated MCP will be submitted 10 days after an option has been exercised. The MCP shall, at a minimum, include required inspections, timelines for inspections, and maintenance to be performed. The Contractor shall perform inspections and maintenance of all VTE in accordance with the MCP. The MCP shall ensure compliance with all minimum code requirements.

3.1.5 The Contractor shall maintain and provide an annual log tracking all PMIs, repairs, and replaced parts. At a minimum, the log will include the date of actions, descriptions of repairs, and part/serial numbers of replaced parts. The running annual log will be provided to the COR as requested and as repairs and inspections are added.

3.1.6 The Contractor shall perform maintenance in accordance with the MCP to ensure reliable and continuous safe operation. The maintenance work shall be in accordance with commercial practices or manufacturer’s specifications and shall be intended to maintain the elevator in a safe and reliable operating condition. The MCP is required to specify appropriate intervals for specific maintenance items.

3.1.7 The Contractor shall perform necessary inspections and tests as required under ASME A17.1 Appendix N, using a Qualified Elevator Inspector (QEI). The first annual test shall include the five-year test for all traction and roped hydraulic elevators regardless of due date.

The three-year test shall be scheduled in conjunction with the annual test two years later from the first annual test. The first test cycle on all elevators shall be completed within thirty days of contract award.

3.1.8 The Contractor shall prepare and submit a written report within two business days of inspection or work to the COR and CO/CA. The report shall identify each elevator, the location, maintenance performed, repairs needed, repairs recommended, date of inspection, name of inspector, and overall condition of the elevator.

3.1.9 The contractor shall post the annual certificate of inspection in the elevator mechanical room and provide two copies (one each, paper and electronic) to the COR and CO within one business day of inspection.

3.1.10 Preventive maintenance shall be performed on all elevators during inspections. The contractor shall provide periodic maintenance reports on all elevators within two business days following inspection to the COR and CO/CA. The scope of preventive maintenance shall be IAW MCP and Task 1.

3.1.11 Cleaning of equipment spaces and daily cleanup of job sites in conjunction with maintenance, inspections and tests, and repairs is required.

3.1.12 The Government may perform inspections of the elevators at no cost to the contractor for purposes of capital asset management and quality assurance. If discrepancies are discovered during these inspections, the Contractor will be notified in writing of any determination and may be responsible for the corrective actions.

3.1.13 The Contractor shall maintain a copy of the current VTE inspection certificate/documentation, along with an index indicating the location, date inspected, and date of the next required inspection having provided the original to the CO/COR. The index and copies of certificates shall be delivered to the CO/COR upon request; however, no later than one (1) business day of request to review and shall be turned over in entirety with the request for final payment.

3.1.14 The contractor shall, at no additional charge, verify the safe operation of any elevator following any malfunction or service call.

3.1.15 The contractor shall warranty work completed under this contract for 30 days following the completion of each maintenance service call. During this warranty period, the Contractor shall, at no additional cost to the Government, promptly undertake corrective maintenance to address any recurring faults or deficiencies directly related to the original services performed.

This corrective maintenance includes troubleshooting and repair; however, any required replacement parts beyond those initially installed will be provided at the Government’s expense.

3.1.16 The contractor shall submit a full and complete invoice in Wide Area Work Flow (WAWF) within 30 days of service. The invoice description in WAWF shall include the date(s) the service was provided and quote number.

3.2 Task 2: Service Calls/Routine and Emergency

3.2.1 The Contractor shall provide service calls (routine and emergency) under the full maintenance service agreement at no additional charge. Authorization call list will be provided at contract award. All repair work shall be done in accordance with standard commercial practices. More than two callbacks for the same problem at the same elevator will be considered a quality deficiency. Technicians shall respond as either a routine or emergency, as determined

P a g e 10 | 21 by the Government, at no cost to the Government. The contractor shall repair and replace components of the elevator up to $500.00 at no additional cost to the government. When a component in a labeled product is replaced, it must be replaced with an identical component manufactured under the original labeling service (certifying agency). Substitution of a different component is only permitted where it is equivalent to that which is tested, as determined by the certifying agency. The parts used for replacement are required to comply with all the requirements with which the old parts originally complied. The Unified Facilities Guide Specifications (UFGS) and the UFC will be used to benchmark acceptable replacement components. See UFGS 14 21 00.00 20 Electric Traction Elevators, UFGS 14 21 13 Electric Traction Freight Elevators, UFGS 14 21 23 Electric Traction Passenger Elevators, and UFHS 14 2 00 Hydraulic Elevators for guidance. Any part, component, assembly, unavailable, from the manufacturer due to obsolescence, remains the Contractor’s responsibility to obtain and engineered product from a commercial source

3.2.2 Routine Service Calls. Service calls will be classified as routine when the work or adjustments do not qualify as an emergency call.

3.2.3 The contractor shall respond in-person to the VTE and begin work on routine service calls at no additional cost to the government within 24 hours after receipt of the call on regular scheduled work days/hours. The person responding to the service call shall contact the COR or CO upon arrival and again upon departure from the base. All routine service calls are included under CLINX002. If additional work is required, it will be negotiated under additional CLINS after the initial service call is performed and prior to start of said additional work.

3.2.4 The contractor shall report to the work location, survey the repair, and provide the CO/COR an estimate of time and cost of repairs if repairs exceeds coverage under the full maintenance service contract, but necessary to bring the elevator back to operation, The Contractor shall commence repair work after notification to start from the CO/COR. If work is started without prior approval, the work will be considered fully covered under the full maintenance service contract. The Contractor shall prepare and submit to the CO/COR a written report within two business days after the repair. The report shall include the date and time of the service call, the location of the elevator, the repairs performed, and the name of the technician performing the repairs.

3.2.5 Billing for labor over the two hours provided under CLINX001 shall be under CLINX003. Hourly rate shall include all travel/per diem. Hourly rate will start when the crew arrives on base and will terminate upon departure as verified by the COR or CO. The Contractor shall contact the COR or CO upon arriving at the worksite and departing the worksite.

3.2.6 Billing for additional parts exceeding the $500.00 included under CLINX001 or CLINX002 shall be billed under CLINX004. The Contractor shall provide a cost estimate prior to repair/replace items over $500.00 to the COR and CO in the form of an itemized quote. The Contractor shall support their quote with bids from three vendors for each part over $500.00

P a g e 11 | 21 being repaired/replaced. Upon approval of the cost by the CO via a contract modification, the contractor shall make repairs. Additional work shall be completed with 30 days of issuance of a contract modification. The only exception shall be when proof of extended delivery lead-time is provided to the Government and with Government acceptance.

3.2.7 Cost for any testing or certifications required on the elevator following repairs shall be included as part of the monthly maintenance, repair, and inspection services.

3.2.8 Emergency Calls. Emergency calls are defined as calls made for services when a VTE system fails and constitutes a danger to personnel; threatens to damage properties; or threatens to disrupt activity, operations, and/or training missions.

3.2.9 The contractor shall respond in-person to the elevator and begin work on emergency service calls within 4 hours of receipt of the call. The person responding to the service call shall contact the COR or CO upon arrival and again upon departure from the base. All emergency service calls are included under CLINX002. If additional work is required, it will be negotiated under additional CLINX003 and CLINX004 after the initial service call is performed and additional work is determined to be necessary and work has been authorized by the CO/COR.

3.2.10 The contractor shall report to work location, survey the repair, and provide an estimate of time to repair and cost of repairs if repairs exceed the coverage under the full maintenance service contract but necessary to bring the elevator back to operation. The Contractor shall commence repair work after notification to start from the CO/COR. If work is started without prior approval, the work will be considered fully covered under the full maintenance service contract. The Contractor shall prepare and submit to the CO/COR a written report within two business days after the emergency repair. The repair shall include the date of the service call, the location of the elevator, the repairs performed, and the name of the technician performing the repairs.

3.2.11 Billing for labor over the two hours provided in CLINX002 shall be under CLINX003.

Hourly rate shall include all travel/per diem. Hourly rate will start when the crew arrives on base and will terminate upon departure as verified by the COR or CO. The Contractor shall contact the COR or CO upon arriving at the worksite and departing the worksite.

3.2.12 Billing for additional parts exceeding the $500.00 included under CLINX002 shall be billed under CLINX004. The Contractor shall provide a cost estimate prior to repair/replace items over $500.00 to the COR and CO in the form of an itemized quote. The Contractor shall support their quote with bids from three vendors for each part over $500.00 being repaired/replaced. Upon approval of the cost by the CO via a contract modification, the contractor shall make repairs. Additional work shall be completed with 30 days of issuance of a contract modification. The only exception shall be when proof of extended delivery lead-time is provided to the Government and with Government acceptance.

3.2.13 Any testing or certifications required on the elevator following repairs shall be included as part of the monthly maintenance, repair and inspection services.

4. Quality Control / Quality Assurance

4.1. Post Award Conference / Periodic Progress Meetings: The Contractor agrees to attend any post award conference convened by the contracting activity or contract administration office in accordance with Federal Acquisition Regulation Subpart 42.5. The Contracting Officer (CO), Contracting Officer’s Representative (COR), and other Government personnel, as appropriate, may meet periodically with the contractor to review the contractor's performance. At these meetings, the contracting officer will apprise the contractor of how the Government views the contractor's performance and the contractor will apprise the Government of problems, if any, being experienced. Appropriate action shall be taken to resolve outstanding issues. These meetings shall be at no additional cost to the government. Meetings may be held in person, by conference call, or by email as determined by the CO.

4.2. Contract Manager (CM): The contactor shall designate a CM who shall ensure performance under this contract. The name of this person, and an alternate who shall act for the contractor when the CM is absent, shall be designated in writing to the CO within 30 days of contract award and 10 days after exercise of option. The CM or alternate shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract. The CM shall work through the COR to resolve issues, receive technical instructions, and ensure adequate performance of services. The CM shall ensure that contractor employees do not perform any services outside the scope of the contract without an official modification issued by the CO. The CM shall ensure contractor employees understand that services performed outside the scope of the contract are performed wholly at the expense of the contractor.

4.3. Quality Control (QC): Contractor shall develop and maintain a quality control program (QCP) to ensure maintenance and repair services are performed in accordance with ANSI/ASME A17 and other applicable standards and codes. The Contractor shall develop and implement procedures to eliminate reoccurrence of once identified/repaired defects. As a minimum, the Contractor shall develop quality control procedures that address the areas identified in Technical Exhibit 1, Service Summary. The QCP shall demonstrate how the Contractor ensures quality performance during the contract period of performance. The Contractor shall maintain the QCP throughout the period of performance of the contract. The CO will be provided updates to the QCP as they occur during the period of performance but at least within 30 days of contract award and within 10 days of option exercise. The QCP will identify the procedures in writing for inspections, individual responsible, VTE MCP, and the location of all inspection records and key control logs, which will always be available to the government upon request. The QCP will have the inspection forms and records used for the service. The Contractor will identify to the CO/COR the responsible quality control inspector to notify in case of customer complaints.

4.4. Quality Assurance (QA):

4.4.1. COR Surveillance with QASP: The Government will periodically evaluate the contractor’s performance under this contract in accordance with the Quality Assurance Surveillance Plan (QASP). This plan is primarily focused on what the Government will do to ensure that the contractor performs in accordance with the performance standards. It defines how the performance standards will be applied, the frequency of surveillance, and acceptable quality level(s) or defect rate(s). The COR is the primary contact for surveillance and contract performance. The COR does not have the authority to make any contract changes or authorize additional work outside the scope of this PWS.

5. Government Furnished Property, Material, Equipment, and Services (GFP/M/E/S)

5.1. General: The Government will provide the property, material, equipment, and/or services listed below solely for the purpose of performance under this contract.

5.2. Workspace: Workspaces for this contact are elevators, elevators shafts, mechanical rooms, and the immediate vicinity of these areas.

5.3. Utilities: The Government will furnish utilities when available for contractor to perform services as described in this PWS. The contractor shall conserve use of utilities at all times.

Electricity, sewage, and water will be made available where existing. No temporary connections will be allowed until the contractor has submitted a detailed plan for the connection and has received approval from the Government for the connection. The plan shall include a detailed description on how the connection will be restored to its original condition or better. Any alterations or temporary connections to Government utilities shall be restored to original condition prior to base departure. Should the contractor require higher voltage or amperage other than available at the building, or if continual tripping of circuit breakers is due to load draw, the contractor may be required to provide their own power source at no additional cost to the Government.

5.4. Emergency Medical Service: In the event of an emergency, Contractor employees should dial 911 and tell them you are calling from Whiteman Air Force Base. Ambulance transport responds from Whiteman AFB Clinic, Monday through Friday. On weekends and holidays, Warrensburg ambulance transport will respond. One of the units will respond to the 911 call and, if necessary, transport a Contractor employee to one of the local hospitals. The Contractor employee shall reimburse the respective agency for these services.

5.5. Smoking Areas. While on Whiteman AFB, smoking is only permitted in designated smoking areas. Contact the building facility manager and/or Government representative for areas where smoking is permitted.

Contractor: A supplier or vendor awarded a contract to provide specific supplies or service to the government. The term used in this contract refers to the prime.

Contracting Officer (CO): A person with authority to enter into, administer, and or terminate contracts, and make related determinations and findings on behalf of the government. Note: The only individual who can legally bind the government.

Contracting Officer Representative (COR): An employee of the U.S. Government designated by the CO to monitor contractor performance. Such appointment will be in writing and will state the scope of authority and limitations. This individual has authority to provide technical direction to the Contractor as long as that direction is within the scope of the contract, does not constitute a change, and has no funding implications. This individual does NOT have authority to change the terms and conditions of the contract.

Deliverable: Anything that can be physically delivered and includes non-manufactured things such as certificates, MCP, schedules, invoices, quotes, meeting minutes, or reports.

Key Personnel: Contractor personnel that are evaluated in a source selection process and that may be required to be used in the performance of a contract by the PWS. When key personnel are used as an evaluation factor in best value procurement, an offer can be rejected if it does not have a firm commitment from the persons that are listed in the proposal.

Physical Security: Actions that prevent the loss or damage of Government property.

Quality Assurance: The government procedures to verify that services being performed by the Contractor are performed according to acceptable standards.

Quality Assurance Surveillance Plan (QASP): An organized written document specifying the surveillance methodology to be used for surveillance of contractor performance.

Quality Control: All necessary measures taken by the Contractor to ensure that the quality of an end product or service shall meet contract requirements.

Subcontractor: One that enters into a contract with a prime contractor. The Government does not have privity of contract with the subcontractor.

8.2. PWS Specific Definitions:

8.2.1. Maintenance, Repair and Replacement. Maintenance, repair, and replacement are on existing VTE and included in the contract scope of services. All maintenance, repair, and replacement of damaged, broken, or worn parts are to be done in a manner that ensures that the equipment may be operated safely. Maintenance, repair, and replacement are actions to restore equipment to a state in which it may safely perform its required operations as installed in accordance with the manufacturer’s recommendations. See ASME A17.1 for requirements.

8.2.2. Routine calls are defined as calls made for services when the work or adjustments do not qualify as an emergency call.

8.2.3. Emergency calls are defined as calls made for services when a VTE system fails and constitutes a danger to personnel; threatens to damage properties; or threatens to disrupt activity, operations, and/or training missions.

8.2.4. Alteration. An alteration is also on existing VTE but is not included under the full maintenance service arrangement. Alteration/modernization will be covered on an over and above Contract Line Item or it may result in a modification to the contract. The typical alteration results in a betterment to the safe operation of the equipment. In the case of elevators, when an alteration is made, compliance to all affected safety requirements must be followed. All work as part of an alteration is required to comply with specific requirements of the current ASME A17.1 Code edition. See ASME A17.1 for requirements.
