SUMMARY OF CHANGES
The purpose of Amendment 0009 is to:Block 14 (Description of Amendment/Modification):
- Update FAR clause 52.212-5 to reflect the updated FAR clause 52.222-55, Minimum Wages for Contractor Workers Under Executive Order 14026 (JAN 2022). Vendors can review the full text of FAR 52.222-55 at https://www.acquisition.gov/far/part-52.
- Replace Attachment 2 - Appendix B (27 JAN 2022) with Attachment 2 - Appendix B (1 FEB 2022) in order to correct frequencies of Level 4 services and correct the running track's square footage.
- Replace Attachment 5 - Past Performance Survey (22 Nov 2021) with Attachment 5 - Past Performance Survey (1 Feb 2022) in order to extend the Past Performance Survey (PPQ) submission deadline to match the offer due date/time (see addenda to FAR 52.212-1, paragraph 5.6).
- Add Attachment 12 - Additional Questions and Answers to address questions received in response to this solicitation.

Description/Specifications/Statement of Work
The Requirements text has been modified to:
The Contractor shall provide all management, tools, equipment, and labor necessary to ensure that custodial services are performed at F. E. Warren AFB, Wyoming, in a manner that will maintain a satisfactory facility condition and present a clean, neat and professional appearance.
Notice to Offeror(s)/Supplier(s): Funds are not presently available for this effort. No award will be made under this solicitation until funds are available. The Government reserves the right to cancel this solicitation, either before or after the closing date. In the event the Government cancels this solicitation, the Government has no obligation to reimburse an offeror for any costs.
**Proposals must be valid through 1 August 2022.**

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, as defined in FAR 2.101, on the date of award of this contract, and does not contain the clause at 52.215-2, Audit and Records-Negotiation.
(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR subpart 4.7, Contractor Records Retention, of the other clauses of this contract.
If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.
(e)
(1) Notwithstanding the requirements of the clauses in paragraphs (a), (b), (c), and (d) of this clause, the Contractor is not required to flow down any FAR clause, other than those in this paragraph (e)(1) in a subcontract for commercial items. Addendum to 52.212-1(c), Period for Acceptance of Offers. The paragraph is tailored as follows: The offeror agrees to hold the prices in its offer firm through 1 August 2022.
ADDENDA TO 52.212-1- PROPOSAL PREPARATION INSTRUCTIONS
NOTE: All proposal preparation cost will be the sole responsibility of the offeror. The Government will not reimburse any firm for their proposal preparation cost.
1. To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein.
Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications. Failure to meet a requirement may result in an offer being ineligible for award. The response shall consist of two (2) separately bound parts, Part I - Price Proposal and Part II - Past Performance Information.
2. General Information
2.1. INFORMATION REGARDING SUBMISSION OF PROPOSALS: Hand carried proposals must be delivered to 7505 Barnes Loop, Bldg. 208, F. E. Warren AFB, WY 82005. (E-mailed proposals are preferred but not required). The sealed envelope or package used to submit your proposal must show the time and date for receipt, the Solicitation Number, and the name and address of the offeror. Mailed proposals (USPS, FedEx, UPS, DHL, etc.) must be sent to the following address:

2.2. If the offeror's e-mail attachment size exceeds 20MB, they will exceed current Air Force e-mail attachment size and the email may not be received. If the offeror has files that exceed 20MB, please contact the Contract Specialist, Ms. Marissa Burlando at marissa.burlando@us.af.mil, and the Contracting Officer, Mr. Troy Johnson at troy.johnson.19@us.af.mil for instructions on how to submit files through DoD SAFE, https://safe.apps.mil/.
2.3. Any limitation on the number of proposal pages. Pages exceeding the page limitations set forth in this addendum will not be read or evaluated, and will be removed from the proposal.
2.4. Format for proposal parts shall be as follows:
2.4.1 A page is defined as one face of an 8 1/2" x 11" sheet of paper containing information.
2.4.2. Typing shall not be less than 11 pitch.
2.5. Offerors are cautioned that F. E. Warren AFB has visitor control procedures requiring individuals not affiliated with the installation to obtain a visitor pass prior to entrance. SOME DELAY SHOULD BE ANTICIPATED WHEN HANDCARRYING PROPOSALS. Offerors should allow sufficient time to obtain a visitor pass and arrive at the 90 CONS to the time specified for receipt. Late proposals will be processed in accordance with FAR 52.212-1(f) Late submission, modifications, revisions, and withdrawals of offers.
2.6. Site Visit Information: A Pre-Proposal Site Visit to review selected buildings in Facility Level 1, 2, and all buildings in Level 4 will be held on Wednesday, 15 December 2021 at 1000 hours Mountain Time at F. E.
Warren Air Force Base. All interested parties must submit names of attendees to marissa.burlando@us.af.mil and troy.johnson.19@us.af.mil by 1400 Mountain Time on Monday, 13 December 2021. A Pre-Proposal Briefing will be held at 90 CONS Conference Room prior to the Site Visit. All attendees who do not have access to F. E. Warren Air Force Base, please meet at the Gate 1 off Interstate 25 at 0930 hours Mountain Time for base pass and escort.

3. Specific Instructions
4. PRICE PROPOSAL
Format for price proposal shall be as follows;
4.1. Complete the SF 1449, blocks 12, 17a, 30 (a), (b) and (c). An official having the authority to bind your company contractually must sign and date the SF 1449 Block 30b. Submit one (1) of this part. The SF 1449 must bear an original, authorized signature. In doing so, the Offeror accedes to the contract terms and conditions as written in the Request For Proposal (RFP). All sections of the RFP, excluding Instructions to Offerors and Evaluation Procedures Sections, constitute the model contract.
4.2. Complete all CLINs in the SF 1449 Continuation Sheet Schedule, to include all option periods. To comply with an AF template for this service, price the CLINs. The extended amount must equal the whole dollar unit price multiplied by the number of units. The proposal must be submitted for a base period plus four (4) option periods. Unit prices shall prevail in the event of a mathematical error when calculating the extended price.
4.3. Complete the necessary fill-ins and certifications in provisions. The provision FAR 52.212-3 shall be returned along with the proposal.
4.4. Acknowledge any and all amendments to the solicitation.
5. PAST PERFORMANCE
Provide a list of no more than five (5) of the most relevant contracts performed for Federal agencies and commercial customers within the last three (3) years. Format for past performance proposal shall be as follows;
5.1. Limited to no more than 2 pages per contract listed. Only references for the same or similar type contract desired. Submit one copy.
5.2. Quality and Satisfaction Rating for contracts completed in the past three (3) years: Provide any information currently available (letters, metrics, customer surveys, independent surveys, etc.) which demonstrates customer satisfaction with overall job performance and quality of completed product for same or similar type contract. In addition, explain corrective actions taken in the past, if any, for substandard performance and any current performance problems such as cost overruns, extended performance periods, numerous warranty calls, etc.
5.3. Past Performance Survey (Attachment 5). The Government will evaluate the quality and extent of offeror's performance deemed relevant to the requirements of this RFP. The Government will use information submitted by the offeror and other sources such as other Federal Government offices and commercial sources, to assess performance. Past Performance includes contract efforts that are currently being performed. Relevant contracts include work of a nature, magnitude and scope similar to that required for this contract within the past three (3) years from date of solicitation. 5.4. Past performance regarding predecessor companies, key personnel who have relevant experience, or sub-contractors that will perform major or critical aspects of the requirement may be included as relevant past performance. Furnish the following information for each contract listed:

5.4.3. Contract type
5.4.4. Project title and brief description of contract effort
5.4.5. Period of performance
5.4.6. Contract value at time of award
5.4.7. Current value, including all changes and exercised option
5.4.8. Contracting Officer's name, address and telephone number
5.5. If a teaming arrangement is contemplated, provide complete information as to the arrangement, including any relevant and recent past/present performance information on previous teaming arrangements with same partner. If this is a first time joint effort, each party to the arrangement must provide a list of past and present relevant contracts.
5.6. Each offeror is responsible for sending the attached past performance survey to its chosen references. Upon completion of the survey, they shall be returned directly from the references to the attention of 90 CONS/PKAB, Marissa Burlando/Troy Johnson, 7505 Barnes Loop, Bldg. 208, F. E. Warren AFB, WY 82005. This list may be provided (preferred) by e-mail to marissa.burlando@us.af.mil and troy.johnson.19@us.af.mil. Completed surveys must be submitted no later than Tuesday, 15 February 2022 at 2:00 PM Mountain Time. This requirement for the submission of survey is in addition to the requirement for a list of consecutive contracts.
5.7. Offerors are cautioned that the Government will use information provided by the offerors and information obtained by other sources in the development of the past performance rating.
5.8. Subcontractor Consent: Past performance information pertaining to a subcontractor cannot be disclosed to the prime offeror without the subcontractor's consent. Provide with the proposal a letter from all subcontractors that will perform major or critical aspects of the requirement, consenting to the release of their past performance information to the prime contractor.
