STATEMENT OF WORK
Design-Build (D-B)
Repair Bomarc Dorm, Bldg 13123 Project Location: Vandenberg AFB, CA Base Project Number: XUMU 09-1253B

1.0 INTRODUCTION
This work will be conducted under a Design-Build (DB) delivery method.
2.0 SCOPE OF WORK
2.1 SCOPE & APPLICABLE DOCUMENTS
This Statement of Work (SOW) sets forth the requirements for a design-build construction effort at Vandenberg AFB, CA. The Contractor is to provide the government with a complete engineering design, construction, and warranty for the renovation of the Bomarc Dormitory.
2.2 DESCRIPTION OF WORK
As part of the proposed effort on this contract, the Contractor shall implement the following tasks for the completion of this project.
2.2.1 General Description of Project:
The Contractor shall design and build (D/B) a solution to bring the below issues up to code compliance and to meet all base, federal and user requirements. The work shall be based on, but not limited to, the concept design provided by the 30CONS CO.
Currently, the Bomarc Dormitory, Building 13123, does not meet Air Force and Dorm-4-Airmen Standards. This project requires the Contractor to perform renovation and overall repair of the exterior and interior of the Bomarc Dormitory, Building 13123. GI Java is not included in the project requirements, except that the exterior windows and sliders of GI Java need to be replaced with new windows to match the exterior windows that will be installed in the rest of Bomarc Dormitory. The wall sconces inside GI Java shall also be replaced as part of the Bomarc Dormitory renovation. Replace with LED, dimmable sconces such as Alpha by Edge lighting, 16”, very warm, nickel finish, or equal.
Remove the GI Java awning, patch and repair the walls and columns. Remove GI Java surface wire moulding while ensuring connectivity to all gaming systems. Office Room 109.1 to be storage with finishes the same as original. Add full height wall shelves, hanging board for tools, and a mop sink. See existing storage area in Bedrooms 109A for approx. area required. Bedrooms 109A/B to be Dorm Managers office. See attached plan. Finishes same as original. Remove two closets and one bathroom.
Add door to common area. Reconfigure one bathroom. Add multi-level customer service counter, L-shaped, with cabinets on wall side above and below counter--finishes same as kitchen. Building manager requests that a lockable roof access panel with drop down ladder be installed in order to allow safe, controlled roof access. The Contractor shall provide all investigative, design, demolition, and construction required to bring Building 13123 up to current standards and codes, including but not limited to the codes and standards identified in paragraph 2.3.7. The specific tasks the Contractor shall perform, identified in the attached concept drawings, include, but are not limited to:

a) Repair, paint and reinstall, as needed, metal handrails, guardrails, and metal columns
b) Repair and replace, as needed, building roof
c) Replace wall mounted and ceiling mounted exterior lighting
d) Upgrade the HVAC system, as needed, and commission according to Unified Facilities Guide
Specifications (UFGS) 15995
e) Install central ducted bathroom exhaust systems
f) Provide new signage, as required
g) Provide sound insulation. Note: A minimum rating of R-19 shall be used unless a higher rating is required by design
h) Renovate the fire sprinkler system
i) Replace doors and windows
j) Renovate the interior of the dorm according to Dorm-4-Airmen guidance standards identified in the Unaccompanied Housing Guide, including renovating the kitchen, common area, patio, and bathroom
k) Paint the interior of the dorm rooms
l) Finish the ceiling of the dorm rooms
m) Complete other interior and exterior repairs necessary to bring the dorms up to Vandenberg AFB
Facility Excellence Standards
n) Cabinet next to washer must be large enough to house the vacuum cleaner. (Match Jupiter Dorm for standardization.)
o) Can Lighting must be secured. (Match Jupiter Dorm for standardization.)
p) Prevent mirror medicine cabinets from slamming into main bathroom mirror or other objects.
q) One USB/Power outlet per bedroom as minimum.
r) Thermostats in a common location above main light switch.
s) Install door sweeps/brushes on exterior doors to prevent debris and wind. Door stops shall be wall mounted to protect the sweeps.
t) Bar brackets above the floor (Match Jupiter Dorm for standardization.)
ENVIRONMENTAL REQUIREMENTS:
- Investigate, design and meet any requirements for work.
- Construction General Permit and Storm Water Pollution Plan (SWPPP) required for site disturbance over 1 ac
- Low Impact Development (LID) standards shall be applied
- Prepare Environmental Protection Plan (EPP)
DESIGN REQUIREMENTS:
-All design/construction shall comply with DoD Unified Facility Criteria (UFC) and Unified Facility Guide Specifications (UFGS)
- All Design/construction shall comply with the VAFB Facilities Excellence Standards -All design/construction shall comply with local, State and Federal standards included but not limited to those outlined in attached specifications

2.2.2 Equipment
The Contractor shall ensure all equipment meets the 100% design specifications and codes.
2.2.3 Mechanical
The Contractor shall ensure all mechanical elements meet the 100% design specifications and codes.
2.2.4 Electrical:
The Contractor shall ensure all electrical elements meet the 100% design specifications and codes.
2.2.5 Architectural
The Contractor shall ensure all architectural requirements meet the 100% design specifications and codes.
2.2.6 Environmental
The Contractor shall adhere to all environmental requirements Vandenberg AFB Environmental Specifications Section 01 57 20 in addition to any other environmental coordination and laws applicable.
2.2.7 Work Clearance Request
The Contractor shall adhere to all 30CES Work Clearance Request Requirements. These can be found on the AF Form 332 (Available from 30CONS CO or 30CES PM) and the AF Form 103 (completed by the contractor).
2.2.8 Applicable Documents
The Contractor shall identify and comply with all applicable federal, state, and local statutes; Air Force/ Military instructions, manuals, handbooks, regulations, guidance, and policy letters; Executive Orders ;
UFGS; American Petroleum Institute Codes; National Association of Corrosion Engineers; National Fire Protection; Steel Structures and Painting Council; National Electrical Code; Uniform Fire Code;
Occupational Safety and Health Administration (OSHA) regulations; Environmental Protection Agency regulations; and International Building Code including all changes and amendments in effect on the date of issuance of each contract. It is the Contractor’s responsibility to identify and comply with all applicable and relevant requirements.
The Contractor shall implement this contract in two phases: the design phase and construction phase.
2.3 DESIGN PHASE
The Contractor shall complete development of plans and specifications in accordance with defined levels of effort necessary to construct the project and provide as-built drawings (both electronically and in hard copy) for retention by the Government. The Contractor shall be responsible for implementation, research, coordination, and execution of all regulatory reviews, ensure technical adequacy of the final design, and provide quality control of all phases of the task order. Any concept design provided to the contractor is for reference and design coordination use only. It may not reflect final design conditions and it shall not be assumed to meet all specifications or CAD standards. The Contractor is responsible for all research, interviews, and investigation required for the project.
2.3.1 Project Phasing
The Contractor shall accomplish this in phases as agreed upon by the Base civil engineers (CE) and production staff. All telecoms and meetings shall be coordinated by the Contractor with the CE Project

Manager (PM). The Contractor shall submit meeting minutes within 7 calendar days after each teleconference or meeting. The Contractor shall complete the work as described below.
2.3.2 Site Survey / Charrette
The Contractor shall avail themselves for a design “Charrette” on the day of or the day after the Kickoff Meeting / Site Survey. The design “Charrette” will allow the Government and/or Contractor to discuss any specific concerns not previously addressed. The Contractor shall document decisions and meeting minutes and provide with the 35% Design below.
2.3.3 35% Design The design approach narrative, combined with any provided concept design, and any documentation from the Charrette will be the 35% design. The 35% Design shall include a list of expected Specification Sections that are anticipated, this list does not represent a final list, but allows for initial coordination.
This shall be submitted electronically for record purposes within 21 days of the Charrette. This is not a formal submittal for review, but rather a written submission for use during the 65% Design Review.
2.3.4 65% Design The Contractor shall provide the 65% design for review and comment within 60 calendar days from the design charrette meeting. The 65% design package shall include the drawings, specifications, schedule of material submittals, and design analysis. These documents shall include all major details. The Government shall have 21 calendar days to provide review comments. The Contractor shall schedule and attend review conference at Vandenberg AFB, CA within 7 days of receipt of Government comments.
(1) Design Analysis: The Contractor shall update the Preliminary (35%) Design Submittal by documenting the decision-making process. The Design Analysis shall be bound or in notebook form and include the following:
a. Charrette Documents: The Contractor shall include the Charrette documents as a reference for design review. This shall be an appendix.
b. List of Section Titles and Numbers: The Contractor shall prepare a List of Section Titles and Numbers to be used in the project in accordance with the Master Format - Master List of Section Titles and Numbers from the Construction Specifications Institute (CSI) Manual of Practice.
c. Environmental Surveys: The Contractor shall provide the environmental surveys and include abatement specifications if asbestos, lead-based paint, PCBs, or underground storage tanks (USTs) are affected by work pertaining to any project.
d. Recycled Materials List: The Contractor shall provide a preliminary list of recycled materials that can be used in the project. The Air Force Affirmative Procurement Program (APP) mandates use of recycled materials found in the Comprehensive Procurement Guideline (CPG). The Environmental Protection Agency (EPA) lists the types of products to be included in the APP available on-line at www.epa.gov/cpg/products.htm. This shall be an appendix.

e. Written Explanation of Major Design Decisions: The Contractor shall provide a written narrative describing key design decisions and discussing the engineering reasons behind these. Each decision shall be in its own paragraph or subsection. The discussion shall include at a minimum: why the decision was needed, the decision itself, what the main alternative(s) were, and what code/engineering led to the choice.
f. Executive Summary: The Contractor shall provide a written narrative overviewing the project scope and key project information. The Executive Summary shall be at the front of the Design Analysis. It shall also be provided as a separate PDF file on the submitted discs. The executive summary shall be usable to explain the scope of the project and general level of disruption and impact to base and surrounding locations.
h. Interviews: The Contractor shall document all discussions with the Air Force Project
Manager, using agency, and other base officials including names, position titles, and telephone numbers. Correspondence shall be an appendix. Investigative interviews to determine design requirements etc. shall be included in a separate appendix and referenced in the narrative.
i. Engineering Data: The Contractor shall provide calculations and other supporting engineering data or analyses for equipment or system sizing and shall justify any special requirements that require deviation from normally accepted commercial or Government standards. This shall be an appendix.
j. Manufacturer Data: The Contractor shall provide catalog cuts or other manufacturer data of major equipment items. This shall be an appendix.
(2) Drawings: The Contractor shall submit half size (14” x 20”) copies of updated Drawings plotted on bond. Drawings shall comply with Vandenberg CAD Standards. It is the Contractor’s responsibility to contact the VAFB 30CES Vault after award to obtain these standards.
(3) Specifications: The Contractor shall prepare the Specifications in sufficient detail for a construction contractor to determine the type and quality of materials and methods required for installation of these materials. The Specifications shall be bound or in notebook form and shall follow these guidelines:
a. The Contractor may use any Government or commercial guide specification service or their own specifications; however, the specification format shall strictly follow the Construction Specifications Institute (CSI) Manual of Practice chapters on Master Format - Master List of Section Titles and Numbers, 3-part Section Format, and Page Format.
b. The Contractor shall specify generic characteristics of construction materials and shall avoid the use of proprietary “brand name” specifications, unless Vandenberg AFB requires a specific manufacturer for base-wide compatibility.

c. The Specifications shall be clearly written and shall not conflict with the drawings. The Specifications shall be thoroughly edited so that they do not contain provisions that do not pertain to any project in this task order.
d. The following Vandenberg AFB standard specification sections shall be inserted into the
Specifications for all projects:
− Section 010000 General Requirements − Section 014400 Contractor Quality Control − Section 015720 Environmental Requirements
e. Sections 01000 and 015720 require editing and coordination with the Air Force Project
Manager for specific base requirements and restrictions. Section 01000 provides a project description and other conditions such as occupancy, work schedule, work stoppages, security, utilities, safety, construction and welding permits, and submittals. Section 015720 specifies environmental compliance and monitoring and disposal of removed materials. Special requirements may be added. All changes from the base document must be documented until the 100% when a clean specification shall be provided. Any editing of the 15720 are not valid until expressly approved from the 30CES PM
f. The contractor shall use one or more of the following Vandenberg AFB standard specification sections if asbestos, lead-based paint, PCBs, or USTs exist in any project:
- Section 02075 Removal and Disposal of Asbestos Materials
- Section 02081 Asbestos Abatement – Small Scale
- Section 02082 Asbestos Abatement – Large Scale
- Section 02084 Lead Based Paint (LBP) Abatement and Disposal
g. If applicable, the Contractor shall use the Environmental Surveys to prepare the scope of asbestos, lead-based paint, PCB, and UST abatement and identify the locations of abatement requirements on the Drawings.
h. The Contractor shall become familiar with the Vandenberg AFB standard specification sections to ensure that the remaining specification sections do not conflict with or duplicate these provisions.
i. Section 01440 Contractor Quality Control, or more stringent as approved by the 30CES
PM and the 30 CONS Contracting Officer, will be required at a minimum.
(4) Schedule of Material Submittals: The Contractor shall complete an AF Form 66, Schedule of Material Submittals. The AF Form 66 indicates all material submittal requirements throughout the specifications listed by description of Item, Contract Reference, and Type of Submittal. The AF Form is available in MS Excel.
(5) Recycled Materials List: The Contractor shall provide a final list of recycled materials that can be used in each project. The contractor shall verify the classification of each recycled material by completing a Recovered Material Determination Form available on-line at

(6) Resubmittals: The Contractor may be required to resubmit the 65% Design Submittal if after review the Government determines the submitted design does not meet the requirements set forth in the contract for the 65% design submittal or if it contains sufficient errors that a 95% design could not be achieved without major revisions, as determined by the Government.
2.3.5 95% Design The Contractor shall provide the 95% design for review and comment within 42 calendar days from 65% design approval. Included shall be all drawings, specifications, and design analysis. The 95% design package shall be in a 100% design state with the exception of Government corrections/modifications. The Contractor shall provide the design to the individuals list below. The Government shall have 21 calendar days to provide review comments. The Contractor shall schedule and attend final review conference meeting at Vandenberg AFB, CA within 7 days of receipt of Government comments.
(1) Design Analysis: The Contractor shall make additional corrections from the (65%) Design Review Conference and submit an updated Design Analysis. The Design Analysis shall be bound or in notebook form. In addition to (65%) Design Submittal requirements, the Design Analysis shall include the following:
a. Annotated Review Comments: The Contractor shall annotate the design review comment sheets received at the (65%) Design Review Conference indicating the full disposition of each comment. The Contractor shall provide the reference location where any changes were made to the contract documents.
(2) Drawings: The Contractor shall make corrections from the (65%) Design Review Conference and submit half size (14” x 20”) copies of updated Drawings plotted on bond.
(3) Specifications: The Contractor shall make final corrections from the (65%) Design Review Conference and submit updated specifications. The Specifications shall be bound or in notebook form.
(4) CAD Project File List and Computer Files: The Contractor shall make final corrections from the (65%) Design Review Conference and submit an updated CAD Project File List and Computer Files.
(5) Resubmittals: The Contractor may be required to resubmit the (95%) Design Submittal if all changes from previous submittals have not been incorporated.
2.3.6 100% Design Within 14 days from this review meeting, the contractor shall provide the 100% Final Design package.
Government shall have 14 days to approve the 100% design. This shall reflect fully complete and ready for construction documents. It shall be fully stamped and sealed.

(1) Design Analysis: The Contractor shall make additional corrections from the (95%) Design Review Conference and submit an updated Design Analysis. The Design Analysis shall be bound or in notebook form. In addition to (95%) Design Submittal requirements, the Design Analysis shall include the following:

(3) Specifications: The Contractor shall make final corrections from the (95%) Design Review Conference and submit updated specifications. The Specifications shall be bound or in notebook form.
(4) CAD Project File List and Computer Files: The Contractor shall make final corrections from the (95%) Design Review Conference and submit an updated CAD Project File List and Computer Files.
(5) Resubmittals: The Contractor may be required to resubmit the (100%) Design Submittal if all changes from previous submittals have not been incorporated.
After final acceptance of the (100%) Design Submittal by the Air Force Project Manager, the Contractor shall submit the required originals and number of sets of copies plus all Computer Files.
2.3.7 Verification of Existing Conditions and Site Survey
Use of existing condition data provided by the Air Force conveys acceptance and as such does not relieve the Contractor of liability associated with performance as the Engineer of Record. The Contractor shall be solely responsible for verification and validation of existing conditions, coordination of existing conditions in parallel with proposed requirements, and above and below-grade condition assessment.
2.3.8 Floor Plans/Elevations Design Requirements
The Contractor shall ensure the subject project is compatible with all Federal, UFC, United States Air Force and Vandenberg AFB, CA Guidelines.
2.3.9 Architect of Record
The term “Architect of Record” in the context of this contract shall mean the principal professional Architect or Engineer in the State of California. The Contractor shall serve as the “Architect of Record” and, as such, will ensure that all completed designs and as-built drawings are signed and sealed by registered professionals. The Contractor is accountable for all aspects of the completed design and as-built drawings under all applicable Federal, State and local laws, regulations and base-specific standards.
2.3.10 Hazardous Materials Surveys

The Contractor shall perform an Asbestos Survey and Lead Based Paint Survey prior to the construction phase of this project. This information shall be reviewed and a Hazardous Materials Abatement Plan produced to address these hazardous materials, if found. The Contractor shall conform to the requirements of Vandenberg AFB Specification Environmental Specifications.
2.3.11 Permits
It is the responsibility of the Contractor to identify all permits, clearances and other documents required for construction as part of the design phase of this project. The Contractor shall obtain the identified permits, clearance and other documents prior to construction.
2.4 CONSTRUCTION PHASE
Upon the approval of the Contracting Officer (CO), the Contractor may commence on-site construction activities. The Contractor shall complete the work as described below:
2.4.1 On-Site Field Oversight
The Contractor shall provide the manpower, equipment, material, services, and transportation necessary to ensure oversight services throughout accomplishment of this design/build contract. It is the responsibility of the Contractor to establish and maintain open communications between salient authorities associated with accomplishment of this contract. During all aspects of work both the Site Superintendent (SS) and the Contractor Quality Control Manager (QCM) shall be on site. The SS and QCM shall be two separate individuals unless expressly allowed for otherwise according to the 30CES PM and 30CONS Contracting Officer.
2.4.2 Delivery/Warranty
The Contractor shall complete all inspection and commissioning requirements prior to final inspection.
Following final inspection, the Contractor shall deliver to the Government the signed and sealed as-built drawings in both hard copy and reproducible format. The Contractor shall issue a warranty in accordance with FAR 52.246-21. All parts and product warranties shall be provided with all paperwork completed by the Contractor on the Government’s behalf. At least one copy of the as-built drawings shall a full-size set on Mylar.
3.0 GENERAL REQUIREMENTS
3.1 QUALIFIED PERSONNEL
The Contractor shall ensure that only qualified, competent personnel carry out the tasks outlined in this SOW. Competent is defined as registered professional or, where registration is not applicable, trained, and/or certified in their respective field.
3.2 AIR FORCE PROPERTY
All materials gathered and/or developed in the performance of these tasks listed shall be returned to and become property of the Air Force; and shall not be used and/or distributed by the contractor without specific written permission of the CE PM.
Material resulting from demolition activities shall be disposed of by the Contractor through the 30CES Qualified Recycling Program, the Defense Logistics Agency (DLA) Disposition Services or off base landfill, as appropriate. The VAFB Base Landfill is no longer available for projects at Vandenberg AFB.

3.3 DESIGN AND CONSTRUCTION REQUIREMENTS
The Contractor shall ensure the project conforms to all applicable Federal, State and local laws, regulations and base-specific standards and applicable Unified Facilities Criteria (UFC). Where there is a conflict between the referenced standards, the most stringent shall take precedence. All work accomplished by the Contractor shall comply with these criteria unless a waiver is obtained from the Air Force when conditions warrant such consideration. The Contractor is responsible for all research, interviews, and investigation required for the project.
4.0 GOVERNMENT FURNISHED ITEMS
The following is a list of specific items of support that the Base CE will provide:
None specific to the project.
4.1 AF FORM 103
The Base CES Customer Service (Building 11439) will assist the Contractor with efforts to fully complete the Base Civil Engineering Work Clearance Request (AF Form 103). The Base CES/CEO will assist in locating underground utilities and issue the AF Form 103 to the Contractor. It is the Contractor’s responsibility to have a completed and approved Form 103 prior to beginning any construction activities including clearing, laydown, mobilization, grubbing, exploratory geophysical work, etc.
Coordination of the AF Form 103 requires a physical presence to carry the document around to multiple agencies on VAFB for coordination. AF Form 103s will require the EPP to be completed before the final signatures can be obtained.
4.2 STAGING AREA
The Base CE will identify space on the jobsite or in a base contractor staging area where the contractor may deliver non-hazardous materials within the installation. An approval letter from Base planning is required for laydown areas.
4.3 EXISTING ENGINEERING PLANS, ETC.
The Base CE will provide the Contractor access to any existing, available and allowable engineering plans, drawings, diagrams, aerial photographs, digitized map files, etc., to facilitate evaluation of the site(s), as needed or requested. The primary method of access is for the Contractor to be allowed into the 30CES Design Vault for research on available documents. The Contractor’s use of any existing plans, drawings, diagrams, etc. does not relieve the Contractor from ensuring the design matches existing site conditions or complying with Vandenberg AFB CAD Standards.
4.4 PRECONSTRUCTION MEETING
The Base CE will host a pre-construction (i.e., Kick-Off) meeting to advise and coordinate on local procedures, safety and security requirements, and project coordination activities. The CE PM will arrange for a suitable Kick-Off meeting location with teleconferencing capabilities to ensure CO participation.
The CE PM will ensure the appropriate base agencies attend the Kick-Off meeting to provide necessary information or briefings to facilitate the success of the contract effort.
4.5 BASE REQUIREMENTS
The Base CE will advise the contractor of the location and processes to obtain the following as needed:

a. Personnel identification badges, vehicle passes, and/or entry permits.
b. Information on base fire prevention/security practices and procedures.
c. Access to utilities (electrical, water, sewer, phone, etc.) This includes a supply (e.g., fire hydrant or stand pipe) of large quantities of potable water.
d. Security escorts for work in restricted areas.
e. Safety training classes for the purpose of allowing access within secure or hazardous areas.
f. A paved area where equipment can be cleaned.
g. A set of keys to locks as necessary.
4.6 CIVILIAN ENGINEERING PROGRAM MANAGER (30CES PM)
The Base CE will provide a CE PM who will review and certify the Contractor’s progress by signing AF Form(s) 3064 and 3065 and Monthly Status Reports for the Contractor. (Note: If the CE PM does not agree that the progress is as represented by the Contractor, he/she shall return the documents to the Contractor for correction.) The Government may utilize a Title II inspector to perform on-site inspections on behalf of the CE PM. If used, the Title II inspector does not have the authority to sign or approve work on behalf of the Government or direct the contractor to perform work outside of the terms of the contract.
5.0 DELIVERABLES

5.1 PROJECT MANAGEMENT PLAN

The Contractor shall submit a Project Management Plan that includes a description and flowchart of personnel involved with the project and a thorough description of construction management. The construction management narrative shall include timing of permitting, lead time for material, use date for user, etc.
5.2 ACCIDENT PREVENTION PLAN
The Contractor shall submit an Accident Prevention Plan that analyzes and addresses all safety concerns according to all specifications and other requirements.
5.3 QUALITY CONTROL PLAN
The Contractor shall submit a Quality Control Plan that analyzes and addresses all safety concerns according to all specifications and other requirements.
5.4 ENVIRONMENTAL PROTECTION PLAN
The Contractor shall submit an Environmental Protection Plan that analyzes and addresses all safety concerns according to all specifications and other requirements.
5.5 CONTRACT PROGRESS SCHEDULE (AF 3064)
The Contractor shall submit a signed AF 3064 Contract Progress Schedule, depicting an overall contract progress schedule for the main elements of work for the period of performance as described in the Table of Deliverables above. The Contractor shall also provide a line graph depicting actual construction progress (solid line) versus scheduled construction progress (dotted line) throughout the period of performance. The CE PM will sign the AF 3064 and send to the Contract Specialist for coordination and approval by the CO.
5.6 CONTRACT PROGRESS REPORT (AF 3065)
The Contractor shall submit AF 3065 as described in the Table of Deliverables above. This report shall be signed by the contractor’s on-site representative in the Remarks section of the AF 3065. The work elements and percentages of the total job identified on the AF 3065 shall be identical to the work elements and percentages on the AF 3064. If the Contractor is more than 5% behind schedule, the Contractor shall note in the Remarks section the reason why and what is being done to get back on schedule. The CE PM will sign the AF 3065 and send to the Contract Specialist for coordination and approval by the CO. If the Contractor requires reimbursement of a high dollar value item, the item shall be identified on the AF 3065, an appropriate percentage applied, and the item must be received and stored on site.
5.7 MONTHLY STATUS REPORT (MSR).
The Contractor shall submit Monthly Status Reports that communicate activities accomplished during the previous period, document discussions, identification of problems, proposed solutions, corrective actions taken, outstanding issues, and record issues or concerns. The Monthly Status Report shall include a financial tracking table containing columns with the following headings:

b. Current Project Status (summary of work previously completed and completed during the current reporting period).

An AF 3064 and, AF 3065 signed by the CE PM and CO, and a Monthly Status report shall accompany each invoice. The AF 3064, AF 3065, Monthly Status Report and invoice shall all cover the same period of time.
5.8 KICK-OFF MEETING
It is the responsibility of the Contractor to coordinate the date and time of pre-construction (i.e., Kick-Off) meeting with the Base Project Manager (CE PM, who will arrange for CO participation), reference Para
4.4. During the meeting, the Contractor shall present a complete overview of the project and review the schedule, highlighting any issues (review periods, material submittals, etc.) which are key to maintaining the schedule. The Contractor shall take meeting minutes to include the recording of the attendance list.
5.9 CHARRETTE
The Contractor shall perform a Charrette as described in the Table of Deliverables. The Charrette will allow the Government and/or Contractor to discuss any specific concerns not previously addressed. The Charrette shall not be concluded until the Contractor has a complete, workable design concept.
5.10 PROGRESS MEETINGS
The Contractor shall perform Progress Meetings as described in the Table of Deliverables. These meetings will include all relevant base POC’s and Contracting Personnel on an as needed basis. These meetings will cover the progress made in the past period and will include any pertinent issues which could affect future task order progress. The following items shall be covered:

b. Project Percent Complete (Actual and Scheduled)
c. RFI’s and Statuses
d. Material Submittals and Statuses
e. Three Week Look Ahead
f. Schedule

5.11 FORMAT OF DELIVERABLES
The Contractor shall provide an electronic copy of all deliverables, formatted for use on the latest version and compatible with the Government’s version of Microsoft Word, Excel, Adobe Acrobat, and AutoCAD.
The Contractor shall date all deliverables. The Contractor will be provided samples of the deliverables, upon request.

5.12 DESIGN SUBMITTAL REQUIREMENTS
In addition to the requirements of applicable UFC’s, the Contractor shall include the follow in each design package:
a. Executive summary with statement of work and all modifications.
b. Table of Contents
c. Minutes and sign-in sheets of all meetings pertaining to the development of the design.
d. Previous government review comments and contractor responses.
e. All appropriate Engineering Calculations.
f. Design Drawings. All electronic copies design drawings shall be provided in both PDF and
AutoCAD formats. The AutoCAD drawings shall comply with VAFB CAD Standards.
5.13 PRE-FINAL INSPECTION
The Contractor shall conduct a pre-final walk through inspection with Base personnel and publish the pre-final inspection findings in a pre-final inspection (punch list) report. The Contractor shall include a draft DD Form 1354, Transfer and Acceptance of Real Property to the CE PM for review. The initial draft DD Form 1354 should be submitted to the CE PM at minimum 28 days prior to expected project completion.
5.14 FINAL INSPECTION
The Contractor shall conduct a final inspection with base personnel and publish the findings in a final inspection report. The inspection shall concentrate on the items identified at the pre-final inspection and recorded in the pre-final inspection (punch list) report. A final inspection shall not be performed until the pre-final inspection (punchlist) report has been resolved. At the final inspection, the Contractor shall present a completed DD Form 1354, Transfer and Acceptance of Real Property to the Base Civil Engineer (BCE) or other appropriate organization for signature and acceptance, if required.
