SUBJECT: Contractor Environmental Guide

1. On behalf of 66 Air Base Group/Civil Engineering, Environmental Element, welcome to Hanscom Air Force Base (AFB). We would like to take this opportunity to explain some of the environmental requirements related to performing work on this installation, and also inform you of our Environmental Management System (EMS) requirements.

2. The role your company plays in keeping Hanscom AFB compliant is vital. Adherence to this guide will help increase your company’s awareness of the installation’s EMS and environmental regulatory requirements that impact Hanscom AFB. This guide covers the significant environmental aspects and impacts that contractors often influence at Hanscom AFB. You will find a list of these aspects in Appendix E. After contract award, lease complete and sign Appendix E, and submit it to the contract administrator immediately after the pre-construction meeting.

3. The contract administrator will place a signed copy of Appendix E in the contract folder and forward a copy to the point-of-contact below.

Hanscom AFB Contractor Environmental Guide

Part 1 – Obligations

1.1. All contractors are obligated to comply with federal, state, and local environmental regulations, and Hanscom AFB Environmental Management System (EMS) requirements. Some of the environmental issues to be considered while completing your job at Hanscom AFB include:

- Hazardous material (HAZMAT) usage & management

- Hazardous waste disposal

- Asbestos abatement and disposal

- Lead abatement and disposal

- Recovered materials usage

- Protection of native plants and birds

- Stormwater discharges

- Construction and Demolition (C&D) waste recycling / disposal

- Waste management

- Influence on the installation’s most significant environmental aspects (see Appendix E)

1.2. Prime contractors are obligated to ensure all subcontractors also adhere to environmental regulations.

Part 2 – Responsibilities

2.1. Contractors, subcontractors and concessionaires are responsible for environmental protection while in the process of executing contracts on Hanscom AFB involving, but not limited to:

- New Construction

- Renovations and major repairs on existing structures or systems

- Demolition/removal of old structures or systems

- Performance of contracted maintenance or services

2.2. All contractors, subcontractors, and concessionaires will comply with environmental regulations.

2.3. Pre-work conference: The prime contractor will brief employees and/or subcontractors on environmental regulations and policies as applicable to the project. Provide documentation of briefing to the Contracting Officer prior to any construction.

2.4. Project sites are subject to inspection at any time by the Hanscom AFB Environmental Element (CEIE) to ensure compliance with environmental regulations and practices.

2.5. Contractors must report spills to the Base Fire Department; dial 911

2.5.1. The Base Fire Department will follow the installation Spill Prevention and Response Plan protocols.

2.6. Contractors must coordinate all spills with CEIE for directions regarding disposal of spill residues (781-225-6913/6142).

2.7. Contractors shall take immediate corrective action to correct substandard environmental practices or conditions in order to comply with environmental regulations if informed of a violation.

Part 3 – Summary of Common Environmental Requirements and Contractor Responsibilities:

3.1. The following, organized by the step/action that occurs in a typical construction project, is a list of common Contractor responsibilities. This table is not all inclusive of all environmental requirements that could be encountered during a construction project (for instance the removal of underground storage tanks). If further guidance is needed on a specific construction project, please contact CEIE at 781-225-6142.

Complete the EMS Most Significant Environmental Aspects Awareness Form at Appendix E. Turn form into contract administrator at or before the pre-construction meeting.

Design Stormwater Management

On all design to the maximum extent practicable Redevelopment projects will meet Massachusetts Stormwater

Management Guidelines for new construction and include a 10% reduction in stormwater runoff volume

Follow Massachusetts Stormwater Management Guidelines for Best Management Practice (BMP) design to include permeability (not percolation) testing to determine exfiltration rates and groundwater separation

Stormwater calculations should include reach routing and CB/pipe capacity evaluation

Stormwater design should correct subsurface conveyance piping to replace shallow catch basins (CB) with deep sump (4-feet) CBs and introduce drainage manholes (DMHs) to eliminate CB to CB connections, whenever possible

Stormwater design analysis for pre-and post-development conditions for 2, 10, 25 and 100-year storm events.

Before Construction/Performance

Tree Removal- Protection of the Federally Threatened Northern Long Eared Bat

(NLEB)

Prior to any tree removal As of April 1, 2015, the NLEB was listed as threatened under the Endangered Species Act (ESA). Hanscom AFB is located within a preferred habitat of the NLEB, in a county confirmed for white-nose syndrome (WNS). The removal of any tree requires coordination with CEIE. Tree removal is prohibited from April 15 to August 31 unless specifically authorized by CEIE.

Work Order must be submitted to CE customer service describing what trees to be removed, reason for removal and location of trees.

CEIE and CE approval is required.

Preservation of Natural Resources

Prior to any action within wetlands resources, buffer to wetland resources, Riverfront area, and/or coastal bank

Contractor will perform consultation and secure all necessary permits, and approvals from outside agencies for any work within wetland resources, buffer to wetland resources, Riverfront area and/or coastal bank. Contractor is required to consult with CEIE regarding any potential project impacts to these resources and notify CEIE of project filings with outside agencies including time, location, and attendance of any public hearings.

Permit (RDA or NOI for filing with Conservation Commission) and/or other relevant permits required for the project

Preservation of Cultural Resources

Required when a building or structure is over 50 years of age, approaching 50 years of age, or was built or used during the Cold War.

Also required when archaeological sites or historic trails are encountered.

The Massachusetts’s Historic Commission (MHC) is required to review federal projects that may affect historic or archaeological properties (National Historic Preservation Act; Arizona Historic Preservation Act).

Contact CEIE (781-225-6144) to determine if the building/structure/archaeological site is already classified as a historic site. CEIE will contact the MHC if necessary. This process is typically 35 to 120 days or more.

If no cultural survey exists then one must be completed before construction begins.

Approval must be granted prior to work commencing.

HAZMAT

Review

Always As part of project Bid submission and no later than 10 calendar days prior to anticipated delivery, submit a list of all hazardous material for base approval prior to usage (e.g., paint, adhesives, sealants, paint remover, refrigerants, pesticides, acids, compressed gases, etc.). Submit a Safety Data Sheet (SDS) and contractor AF Form 3952 for each hazardous material to be used during the project.

Refer to Appendix A for guidance

Submit material usage, SDS(s), and AF-Form 3952s to the Contracting Office.

Approval of HAZMAT must be received l prior to HAZMAT delivery to HAFB project site.

Asbestos Determination

Prior to renovation or demolition activities

If a project has already been designed (i.e., it is not a design/build and it is not a SABER project), then CE will inform the contractor of the asbestos determination.

If a project is a SABER project or a design/build project, then the contractor is responsible for the asbestos determination as follows:

The contractor must test for asbestos prior to renovation or demolition activities using an EPA accredited Asbestos Building Inspector.

If asbestos is present and must be abated, the Contractor must:

Submit the following documents prior to Notification to MassDEP:

Asbestos Abatement Work Plan (For approval by Environmental Section (66ABG/CEIE), Bioenvironmental, and CE Safety prior to Notification) Mass. DLWD Asbestos Abatement Contractors License Asbestos Supervisors License Asbestos Workers License Consultant Firms Laboratory License Project Monitors License

Notification to MassDEP Submit to MassDEP at least 10 days prior to start of work Submit copy of notification, including date submitted, to

CEIE.

Submit a copy of the Asbestos Survey Report to the Contracting Office.

If asbestos is present, submit a copy of the abatement plan, contractor/supervisor/ worker licenses, lab license, project monitor license, and notification to MassDEP Contracting Office.

(See Appendix C-2)

The removal of a UST/AST must be with in compliance with storage tank and hazardous waste regulations.

Early during pre-construction, a coordination with local Fire Departments and CEIE is required for the UST or AST removal.

The removal of the UST must comply with 527 CMR 9.00.

Coordination with the local Fire Department (Bedford, Lexington or Lincoln) and CEIE is required prior to commencement of AST/UST removal activities. A permit for removal of UST is required. Contact local fire department for permit and contamination assessment requirements. Refer to Appendix D for guidance.

Submit the tank UST removal permit to the Contracting Office and coordinate with CEIE prior to removing any

UST/AST.

Lead-Based Paint (LBP) Determination (For Non- Residential Facilities)

Testing for LBP is necessary prior to renovation, repair, demolition, sanding, sandblasting, or maintenance activities that will involve or disturb painted surfaces.

If a project has already been designed (i.e., it is not a design/build and it is not a SABER project), then CE will inform the contractor of the lead-based paint determination.

If a project is a SABER project or a design/build project, then the contractor is responsible for the lead-based paint determination as follows:

LBP data may be available for review for some buildings. Contact the Bioenvironmental Engineering Office, CE or CEIE to determine if LBP data already exists for the specific building or work area.

If data does not exist or is inadequate testing for LBP is required

If it is determined that LBP is present above the action level and will be disturbed, the contractor will ensure activities are performed in conformance with regulations governing lead paint in the construction industry (OSHA 29 CFR 1926.62) to ensure occupant, worker, and environmental protection. Necessary abatement and protection procedures shall be incorporated into work plans and contract documents.

Bioenvironmental Engineering and the CE LBP Program Officer review contractor submittals and work plans, including training certifications, for project activities that disturb LBP.

Submit a copy of the LBP Survey Report to the Contracting Office.

If LBP is present, submit a copy of the LBP Removal/ Demolition Activity to the Contracting Officer.

(See Appendix C-3)

Base Civil Engineering Work Clearance Request

(Dig Permit)

Prior to any excavation work, trenching, digging, or boring

Complete Engineering Work Clearance Request – AF Form 103 (Dig Permit) and obtain a dig safe permit.

Submit AF Form 103 and required drawings to Civil Engineering Customer Service (781-225-2990) for processing/approval.

Work shall not begin without an approved AF Form 103.

Construction and Demolition (C&D) Debris Diversion Plan

Always At least 3 weeks prior to the start of construction, submit the C&D Debris Diversion Plan – this plan describes how the contractor will recycle (or dispose) of scrap metal, concrete, wood, green waste, garbage, etc.

Note: DoD Instructions, CFR, Executive Orders, and the Air Force require recycling.

Refer to Appendix B for guidance.

Submit the C&D Debris Diversion Plan to the Contracting Office.

Construction/ Demolition Notification with Mass DEP

Prior to any demolition or construction of a building.

In accordance with 310 CMR 7.09, MassDEP requires notification 10 working days before the start of construction or demolition of a building. This notification requirement is designed to protect public health and the environment by ensuring that the release of dust or other potentially hazardous air pollutants to the ambient air will be prevented. Please note that “working days” are Monday through Friday and do not include Saturdays, Sundays or state holidays.

Submit the completed notification showing receipt from MassDEP to Contracting office 7 working days prior to start of construction or demolition.

Notice of Intent (NOI), Storm Water Pollution Prevention Plan (SWPP Plan) for NPDES General Permit for Discharges from Construction Activities

1 acre or more of soil disturbance

The NPDES General Permit for Discharges from Construction Activities requires the preparation of a SWPP Plan to be included with the submittal of a Notice of Intent (NOI) to EPA at least 14 calendar days prior to commencing earth-disturbing activities for new projects, immediately for existing projects, at least 14 calendar days prior to transfer to new operator, and no later than 30 days after construction commencement for emergency related projects.

Submit copy of the SWPP Plan and NOI to the Contracting Office.

A copy of the SWPP Plan and NOI shall be coordinated with and submitted to CEIE (x5-6144).

During Construction/Performance

Reporting of HAZMAT usage

Always (Monthly for contracts greater than 60 days)

Refer to Appendix A for guidance. Submit usage information to the Contracting Office.

Asbestos Removal

If asbestos was determined to be present (renovation or demolition activities)

The Contractor must:

Follow the requirements of approved abatement plan and

Notification to MassDEP Ensure that asbestos waste is properly managed Notify CEIE 7 Working Days prior to Transportation of

Waste Provide the CEIE a copy of the Waste Shipment Record

(Signed by Generator and Transporter) the day the any waste is transported of the installation.

Refer to Appendix C-2 for guidance

Submit Waste Shipment Records to Contracting Office and provide CEIE a copy at the time waste is transported from the installation.

Proper management of waste and recyclables (hazardous waste, universal waste, scrap metal, wood etc.)

Always Refer to Appendix C for guidance regarding Hazardous Waste and Universal Waste.

For C&D waste follow the previously submitted C&D Debris Diversion Plan (refer to Appendix B).

C&D logs shall be submittals as described in Appendix B

Any Bill of Lading or Hazardous Waste Manifest for Federal or State Regulated Hazardous Waste or Universal Waste must be signed by a designated person in CEIE. Specific Submittals are listed in Appendix C.

Reporting of recycling and disposal of construction and demolition debris

Always (Monthly for contracts greater than 60 days)

Refer to Appendix B for guidance. Submit monthly recycling and disposal numbers to the Contracting Office.

Proper management of wastewater

Always The base maintains a discharge permit with the MWRA. Ensure any discharge to the sewer meets minimum discharge limits.

Discharge meeting the Stormwater exception rules should not be discharged to the sewer. Contact CEIE (781-225-6144) for more information.

N/A

Preservation of Cultural Resources

When cultural resources are discovered on work site or during construction.

Stop construction immediately and contact CEIE at 781-225- 6144/6142. CEIE will contact the State Historic Preservation Officer (SHPO) for guidance on how to proceed. Additional archeological surveys and testing may be required. The SHPO will review and approve all steps to mitigate the discovery.

SHPO must be contacted through

CEIE.

The removal of UST/AST must proceed in compliance with storage tank and hazardous waste regulations.

Prior to transport of any waste determined to be hazardous.

Refer to Appendix C for guidance regarding Hazardous Waste

Refer to Appendix D for guidance regarding UST/AST Removal

Any Bill of Lading or Hazardous Waste Manifest for Federal or State Regulated Hazardous Waste must be signed by a designated person in CEIE. Specific Submittals are listed in Appendix C and Appendix D.

After Construction/Performance (Prior to Project Close-out) Recovered Materials Certification Upon Completion of Work

When the total contract cost is greater than $100,000

Certification statement by the contractor indicating the use of Recovered Materials as required by the contract specifications

(FAR 52.223-9).

Submit signed Certification Statement to the Contracting Office.

Notice of Termination

1 acre or more of soil disturbance

After completion of construction project, in fulfilment of the requirements of the NPDES permit, the contractor must submit a Notice of Termination (also known as a NOT) to MassDEP.

Submit NOT to MassDEP Submit copy to the Contracting Office.

Reporting of hazardous material usage

For contracts less than 60 days, report total hazardous material used during the project. For contracts greater than 60 days, report the final month’s hazardous material usage

Refer to Appendix A for guidance.

Submit usage information to the Contracting Office.

Removal of

HAZMAT

Always upon job completion. Removal of all HAZMAT brought on site upon job completion.

CE Project Manager and HAZMAT Program Manager to verify HAZMAT removal as part of Cleanup and project closeout.

Submit written verification of HAZMAT label(s) turn-in and site Cleanup to Contracting Office

Hazardous Waste Manifest Signed by Receiving Facility

For ALL Hazardous Waste Manifests

Within 35 days after the waste leaves Hanscom AFB:

o Copy of the manifest that is signed by the receiving facility must be provided to Hanscom AFB, CEIE.

o If over 45 days, CEIE must contact regulatory agencies.

Submit manifest signed by receiving facility to Contracting Office and CEIE within 35 Days after the waste leaves the installation

Asbestos Waste Shipment Record (WSR) Signed by Receiving Facility

For ALL Asbestos Abatements Within 35 days after the waste leaves Hanscom AFB:

o Copy of the WSR that is signed by the receiving facility must be provided to Hanscom AFB, CEIE.

o If over 45 days, CEIE must contact regulatory agencies.

Submit WSR signed by receiving facility to Contracting Office and CEIE within 35 Days after the waste leaves the installation

Tracking of UST Disposal Receipt and Signed Hazardous Waste Manifest

After UST/AST is removed UST disposal receipt and hazardous waste manifests (if applicable) must be provided to CEIE.

Submit UST Disposal Receipt and manifest signed by receiving facility to Contracting Office and CEIE within 35 Days of transport.

GUIDANCE FOR SUBMISSION OF SAFETY DATA SHEETS

FOR HAZARDOUS MATERIAL (HAZMAT) AND REPORTING OF HAZMAT USAGE

Hazardous Materials: A hazardous material is commonly known as a HAZMAT. OSHA defines a hazardous material as any substance to which exposure “results or may result in adverse effects on the health and safety of employees” or “any chemical which is a physical hazardous or health hazard.” There is no ‘one simple list’ of hazardous materials.

Listed below are the general categories of hazardous materials that require submission of hazardous material usage reporting to the Contracting Office as part of the Bid submission package:

Compressed gases (all types) Adhesives Sealants Paints (including aerosols) Lubricants / oils / fuels Welding materials Solvents Chemicals used in testing or as additives Any fluids (except water) added to machinery / government-owned or leased equipment Pesticides/insecticides/rodenticides

HAZMAT cannot be brought or stored on HAFB without prior notification and approval through the Enterprise Environment, Safety, and Occupational Health Management Information System (EESOH- MIS). HAZMAT purchased by the contractor to fulfill contract requirements are brought on base as needed, used only by the contract employees and are removed from the base at the end of the term of the contract. Excess and unused HAZMAT at project completion will be removed from the base by the contractor.

For all contractor-owned HAZMAT, Part I of an AF Form 3952 (available at HAZMART) must be prepared and a Material Safety Data Sheet (or Safety Data Sheet) must be provided to the Contracting Office. The AF Form 3952 will provide the HAZMAT Program Manager a list of the materials, container sizes, quantities and corresponding MSDSs (or SDSs) for each HAZMAT product to be used to meet contract requirements. The completed Part I of AF Form 3952 and MSDS (or SDS) should be submitted to the Contracting Officer as part of the Bid package and no later than 10 calendar days prior to anticipated delivery of listed items on to HAFB.

When the AF Form 3952 and MSDS (or SDS) is submitted to the Contracting Office, HAZMAT Program Manager will process the HAZMAT approval in EESOH-MIS and assign a shop specific label for each container. Tracking labels are generated within one to two working days of completion of HAZMAT approval through EESOH-MIS. Each contractor’s HAZMAT container stored or used on base for must be labeled with HAZMAT bar-code tracking labels, provided by HAZMAT Program Manager. The contractor will ensure that labels are placed on these containers meeting the requirements of 29 CFR 1910.1200(h), AFI 90-821, Hazard Communication and 40 CFR 262.32 and that management of HAZMAT used on a project meets HAFB Hazardous Management and Operating Procedures (HMOP).

The contractor must turn-in the label to HAZMAT Program Manager when a HAZMAT container is empty or removed from the installation. The contractor shall establish a centralized location to store, receive, transfer, and issue contractor-owned HAZMAT in accordance with all applicable regulations.

The contractor will store no more than the authorized quantity of HAZMAT on site.

The contractor is responsible for furnishing all equipment necessary to manage contractor-owned HAZMAT. This equipment includes, but is not limited to, approved chemical security cabinets, locks, secondary containment, spill response equipment, fire extinguishers, personal protective equipment, and computer equipment.

All HAZMAT must be removed from the base at the end of contract activities or the end of the performance period, whichever comes first. The contractor must turn-in the HAZMAT container labels to HAZMAT Program Manager. The Contractor must notify CE Project Manager and HAZMAT Program Manager when the product has been removed so final site inspection can be performed as part of Cleanup to verify material removal. Once this action is completed and verified by HAZMAT Program Manager, CE Project Manager will notify, in writing, the Contracting Office to proceed with final payment and project closeout.

For those hazardous materials that will be tracked the following requirements apply:

a. For contracts less than 180 days, the Contractor will submit the “Contractor Hazardous Material and Usage Data” (refer to Attachment A-2) to the Contracting Office Representative prior to the start of the contracted work, and again at the completion of the contract.

b. For contracts greater than 180 days, the Contractor will submit the “Contractor Hazardous Material and Usage Data” (refer to Attachment A-2) to the Contracting Office Representative prior to the start of the contracted work, once each month throughout the duration of the contract, and at the completion of the contract.

If the contractor needs to bring hazardous material onto the installation that was not included in the original submittal, the contractor must repeat the process discussed above.

APPENDIX B

CONSTRUCTION AND DEMOLITION (C&D) DEBRIS

HAFB is required to report the quantity of waste landfilled and the quantity of waste recycled on a quarterly basis. Because most Construction and Demolition (C&D) must be recycled in accordance with Massachusetts waste bans, HAFB tracks the recycling of C&D debris.

Types of C&D debris include, but are not limited to:

Aluminum cans (e.g., soda cans) Asphalt Bricks Cardboard Concrete Drywall Electrical wiring and components Green waste (palm fronds, scrub brush, land clearing debris, etc.)

Landscaping stone Plaster Plumbing fixtures and piping Roof coverings (e.g., shingles or tiles) Metal scraps (from stud trim, ductwork, rebar, piping, roofing, banding, etc.)

Wood (scrap wood)

At least 3 weeks prior to the start of construction, submit the C&D Debris Diversion Plan to the Contracting Office. This plan describes how the contractor will recycle (or dispose) of C&D debris (refer to Attachment B-1 for an example C&D Debris Diversion Plan).

For contracts less than 60 days, the Contractor will submit the “Summary of C&D Debris Recycling and Landfilling” (refer to Attachment B-2) to the Contracting Office prior to the start of the contracted work, and again at the completion of the contract.

For contracts greater than 60 days, the Contractor will submit the “Summary of C&D Debris Recycling and Landfilling” (refer to Attachment B-2) to the Contracting Office prior to the start of the contracted work, each month of the contract, and at the completion of the contract.

It is acceptable to combine the table included in the C&D Debris Diversion Plan and the monthly reporting requirements into one, comprehensive table (as shown in Attachment B-2).

HAZARDOUS, PCB, UNIVERSAL, AND ASBESTOS WASTE:

PROPER MANAGEMENT AND DISPOSAL

Hazardous Waste, PCB, and Universal Waste: The Contractor is required to manage and dispose of hazardous waste, PCB waste, and universal waste in accordance with local, state, and federal laws. It is the Contractor’s responsibility to determine whether any of these wastes will be generated by a particular project and to properly manage such wastes.

Listed below are examples of wastes which can be hazardous waste (this list is not all inclusive):

Acids or caustics Adhesives, caulks, roofing cements Empty aerosol cans Items that contain lead (such as lead flashing, lead solder, lead-based paint chips) Items that contain mercury (such as mercury switches, mercury thermostats, fluorescent bulbs) Paints, varnishes, and sealers Pesticides, insecticides, and rodenticides Waste solvents, thinners, cleaners, and fuels Sandpaper, sand blasting residue Rags, gloves, coveralls, masking paper contaminated with hazardous materials.

The following table presents a list of questions that the Contractor may use to help identify whether their work will be generating a hazardous, PCB, universal waste, or asbestos waste (these questions are not all inclusive).

Many solvents (e.g., chlorinated solvents, benzene, toluene, MEK, xylene, etc.)

are hazardous and the stripping residues along with rags and applicators may be RCRA listed spent solvent wastes.

Corrosive strippers (acids, alkalis, etc.) may be RCRA corrosive if disposed in liquid form.

Depending on what is being removed, the residues may also contain RCRA toxic heavy metals.

Must be removed by licensed hazardous waste transporter and accompanied by a hazardous waste manifest.

Empty aerosol cans are regulated as RCRA reactive.

Will any fluorescent lamps be removed?

Fluorescent lamps contain mercury and are regulated as Universal Waste (No Bulbs may be disposed of in trash receptacles). All Universal Wastes must be disposed of in accordance with 310 CMR 30.000.

Will any electronic equipment be removed (equipment containing circuit boards, switches, thermostats, transformers, soldered parts, CRTs, etc.)?

Some types of equipment may contain heavy metals and be RCRA toxic for mercury, lead, or silver.

Transformers and lamp ballasts may contain Polychlorinated Biphenyls (PCBs).

Some types of equipment may contain batteries that have RCRA toxic metals or are RCRA corrosive or are regulated as Universal Waste.

RCRA and PCB wastes must be removed by licensed hazardous waste transporter and accompanied by a hazardous waste manifest.

APPENDIX D

UNDERGROUND STORAGE TANK (UST)/

ABOVEGROUND STORAGE TANK REMOVAL

Removal of Underground Storage Tanks (UST)

The removal of the UST must be within compliance with 527 CMR 9.00. Coordination with the local Fire Department (Bedford, Lexington or Lincoln), along with the environmental office is required prior to commencement of UST removal activities. A permit for removal of UST is required. Contact local fire department for permit and contamination assessment requirements. Proper disposal of the UST, piping, along with any other demolition material associated with this project is required; this includes cleanout, transportation, and appropriate disposal of UST at an approved tank yard (if metal). Any disposal of state regulated or hazardous wastes must be coordinated with the Environmental Section (CEIE). Any hazardous waste manifests must be signed and tracked by CEIE. Also, a disposal receipt from the approved tank yard must be provided to CEIE.

Civil Engineering or its subcontractor is responsible for obtaining the removal permit (including payment of associated fee), conducting contamination assessments, transport/disposal of regulated waste (if applicable), and disposal of UST.

The removal permit, UST disposal receipt, any laboratory analysis (if applicable), and hazardous waste manifests (if applicable) must be provided to and signed by CEIE.

Removal of Permitted Aboveground Storage Tanks (AST)

Coordination with the local Fire Department (Bedford, Lexington or Lincoln), along with CEIE is required prior to commencement of AST removal activities. AST must be properly disposed at an approved tank yard. Any disposal of state regulated or hazardous wastes must be coordinated with CEIE. Any hazardous waste manifests must be signed and tracked by CEIE.

Civil Engineering or its subcontractor is responsible for transport/disposal of regulated waste (if applicable) and disposal of AST.

Any laboratory analysis (if applicable) and hazardous waste manifests (if applicable) must be provided to and signed by CEIE.

APPENDIX E

ENVIRONMENTAL MANAGEMENT SYSTEM (EMS)

MOST SIGNIFICANT ENVIRONMENTAL ASPECTS AWARENESS FORM

Each of the installation's activities and services are reviewed to identify associated environmental aspects. The environmental aspects identified for each activity and service is then summarized for the entire installation.
