NOTES:
1. Offerors' format is acceptable as long as it includes all the required elements within the templates and examples; Microsoft Word or Excel may be used
2. Ensure Labor Categories/Position Titles and #'s match Staffing Plan Matrices and what is proposed
3. Offerors' format must adhere to the font and formatting instructions in the RFP
