Attachment 12 - Past Performance Information Citations -- Unique Areas Front Range Multiple Award Construction Contract (FRMACC)

Identify the "unique areas" within each past performance information citation. You may select multiple "unique areas" per citation. If a particular citation does not contain a "unique area," select "N/A." For Phase One, only information contained within the offeror's past performance citations will be evaluated for unique areas. Phase One will be evaluated on a pass or fail basis. Only offerors that pass Phase One will be considered for Phase Two. See Section M, C., c., ii. Relevancy, for further details.
