About this file
This document is a Past Performance Survey form for a VA lease solicitation (Solicitation Number 36C25025R0020) for a Community Based Outpatient Clinic in Georgetown/Mt. Orab, Ohio. The survey is designed to comprehensively evaluate a lessor's performance across multiple dimensions including management of leased space, customer satisfaction, timeliness, and overall performance.
The survey provides a detailed rating system with five performance levels (Exceptional, Satisfactory, Marginal, Unsatisfactory, and Neutral) for evaluating specific lease performance elements. These elements include staff qualifications, building maintenance, ground/parking conditions, service provision, construction quality, responsiveness to service requests, communication, issue resolution, and project management. The form allows the government evaluator to provide narrative feedback on the lessor's strengths, opportunities for improvement, compliance difficulties, and overall recommendation for future solicitations. The document is intended to be submitted via email to specific VA contacts and serves as a critical tool for assessing past performance in the lease acquisition process.
