About this file
This is an Installation Floater Affidavit form required by Volusia County, Florida for Invitation for Bid 26-B-59IF, which concerns the reconstruction of the El Portal Vehicular Beach Access Ramp in Daytona Beach Shores. The affidavit serves as documentation that a contract bidder's insurance broker has made good-faith efforts to obtain Installation Floater or Builder's Risk insurance with flood coverage extensions, including coverage for storm surge-related flooding, which is a mandatory insurance requirement under Exhibit A of the ITB. The form requires the affiant (insurance broker) to certify contact with at least three insurance companies and document their inability or refusal to provide the specified coverage. The non-mandatory pre-bid meeting is scheduled for March 16, 2026, questions are due by March 26, 2026, and bids must be submitted electronically by April 9, 2026, at 3:00 p.m. The project involves reconstruction of beach access infrastructure destroyed during Hurricane Nicole in 2022, with an estimated construction cost of $1,464,420.00 and a 150-day completion period (120 days for substantial completion).
The affidavit functions as a waiver mechanism allowing bidders to proceed without the specified insurance coverage if they can demonstrate that such coverage is unavailable in the commercial insurance market. This document is critical for the project given its coastal, hurricane-prone location and the specific flood-related risks associated with beach and intracoastal waterway construction. The project is federally funded through FEMA and subject to strict compliance requirements including Davis-Bacon Act wage standards and domestic preference provisions for materials. Award will be determined by lowest responsive and responsible bidder, with contractors required to submit a bid bond equal to 5 percent of the total bid price and, upon award, provide Payment and Performance Bonds equal to 100 percent of contract price. Liquidated damages are assessed at $1,000 per day for failure to meet completion deadlines, and payment will be made within 20 to 25 days of invoice receipt.
