About this file

This is a Prime/Subcontractor Form for Contract No. C-2014 ITB No. 25-B-##XX related to the El Portal Vehicular Beach Access Ramp project in Florida, a reconstruction effort following Hurricane Nicole in 2022. The project involves restoring safe pedestrian and vehicle access to the beach through construction of composite sheet pile seawall panels, concrete cap, reinforced concrete vehicular ramp, curbing, pedestrian sidewalk, and site restoration. Bidders must complete this form to list all proposed subcontractors and are required to identify the dollar value and percentage of work to be performed by the prime contractor and each subcontractor across specified disciplines including demolition and hauling, site security fencing, excavation and grading, coffer dam and dewatering, sheet pile installation, concrete, pavement paint and marking, signage, testing, and surveying. Failure to complete and return this form may result in a non-responsive bid. The prime contractor is responsible for ensuring all subcontractors hold appropriate licenses, and any changes to subcontractors during project completion require prior approval from the County Project Manager.

The form requires bidders to allocate 100 percent of the total bid price across all listed work categories, with costs distributed between the prime contractor and subcontractors. The total bid dollar value must match the bid price listed on the associated Bid Form and Bid Price Sheet. The County reserves the right to seek clarification regarding subcontractors but will not allow changes to the total submitted base bid pricing. This form is also used to determine local preference in accordance with Section 2.8, D. Local Bid Preference of the solicitation. The bidder must clearly identify the company name, complete address, dollar value, and percentage of work for both the prime contractor and each subcontractor, with the form automatically calculating percentages based on entered dollar amounts.
