About this file
This is a Past Performance Questionnaire (PPQ) template for the TRICARE Medicare Eligible Program (TMEP2) federal contract opportunity HT940226R0001. The document is designed to be completed by clients or customers who have worked with contractors or subcontractors to evaluate their past performance for source selection purposes.
The questionnaire requires completion of administrative information including the offeror/team name, contractor/subcontractor identification (DUNS number, CAGE code), contract details (contract number, task order number, contract type, total value, period of performance), and client contact information. Respondents must describe the magnitude of services and tasks performed in relation to RFP criteria section M.7.3.2. The client portion of the PPQ consists of ten evaluation questions assessing contractor performance across multiple dimensions. Questions 1-3 evaluate performance against stated contract requirements, timeliness/processing standards, and pricing compliance. Questions 4-7 assess the manner of work performance, including staff attitude, knowledge/skills/abilities, client satisfaction capability, and responsiveness to problem resolution. Each question uses a five-point rating scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory) with space for detailed comments. Questions 8-10 address transition-in phase performance (data migration, system connectivity, security certifications), willingness to do future business, and additional comments. The questionnaire must be submitted as a PDF email attachment to addison.t.sexton.civ@health.mil and david.c.smith139.civ@health.mil, with responses treated as confidential source selection information.
