About this file
This is a template form for documenting prior contract experience as part of a federal solicitation response.
Part 1 requires offerors to submit no more than three (3) prior projects or contracts completed within five years of the solicitation issue date. For each project, offerors must provide: company name, project/contract title, contract number, original and current/final contract values, completion dates (original, current schedule, and estimated/final), and primary causes for any contract modifications. Offerors must describe the work performed and explain its relevance to the solicited project, identifying specific elements performed by their firm versus subcontractors. The form requires disclosure of major subcontractors and whether they will be used on the new award, along with their principal areas of work. Offerors must also identify any uniquely relevant technical areas and provide points of contact from the government agency, including current phone numbers and email addresses for the Contracting Officer and/or Contract Officer Representative (COR).
Part 2 requires offerors to name subcontractors and teaming partners to be utilized on the contract, with a maximum of three (3) subcontractors permitted per discipline. Specialty trade contractors must identify major subcontractors or teaming partners specific to their trade. Upon completion of Part 1, offerors must prepare and send an Exhibit C Package to the references for each of the three construction projects or specialty trades listed.
