About this file
This is a Certified Drug-Free Workplace Report Coversheet issued by the State of West Virginia Purchasing Division, created on 07/01/13, in accordance with West Virginia Code § 21-1D-7b. The document requires contractors to submit a detailed report to the public authority that issued the contract, either annually or upon project completion. The report must include specific information such as education and training service details, the name of a certified drug testing laboratory, average number of employees for the construction project, and comprehensive drug test results across four categories: pre-employment/new hires, reasonable suspicion, post-accident, and random testing.
The coversheet provides instructions for submission based on contract value, with reports for contracts over $25,000 to be mailed to the West Virginia Purchasing Division in Charleston, and reports for contracts $25,000 or less to be sent to the specific public authority issuing the contract. The form requires vendors to complete contract identification details, check boxes confirming inclusion of required report content, and provide vendor contact information. This standardized reporting mechanism ensures compliance with state regulations regarding drug-free workplace requirements for contractors working on public improvement projects.
