About this file

This is a Budget and Narrative Template for the Opioid Settlement Community Grants administered by the Oregon Health Authority (OHA). The template is designed for applicants seeking funding to address the opioid crisis through one of five categories: Primary Prevention, Overdose Prevention and Risk Reduction, Treatment, Recovery, and Innovative Research. Applicants must complete a comprehensive budget covering the period of July 1, 2027 through June 30, 2029, with flexibility to complete project activities at any time before the end date. The budget template requires detailed line-item entries across nine budget categories, including salary, fringe benefits, equipment, capital expenses, supplies, travel, other expenses, and contracts. Applicants must provide narrative descriptions explaining how each budget item supports project activities and outcomes.

The Opioid Settlement Community Grants are available in two funding tiers: Tier 1 ($25,000–$75,000) and Tier 2 ($100,000–$500,000). Allowable costs include personnel, materials, supplies, capital expenditures up to $250,000, interpretation or translation services, outreach and engagement expenses including food and childcare, contractor services, training, and travel. Indirect costs are capped at 15% of total direct costs. Unallowable costs explicitly exclude overdose reversal medications and harm reduction supplies (provided through the Save Lives Oregon Clearinghouse), major construction projects, expenditures exceeding $250,000, program duplication or supplantation, and medical or behavioral health services billable to Medicaid or Oregon Health Plan. All direct costs must be reasonable, necessary, and directly attributed to approved project activities, with adequate documentation required to support expenditures.

Budget Categories Instruction / Description

(1) SalaryProvide the estimated salary cost of each position being funded by this request. This is only Salary costs, so exclude fringe or benefits from this estimate. Specifics below.
Position #:This field is used to list all the positions you are requesting funding for. Each separate position belongs on a different row.
Title of Position:What is the official title of the position you are requesting funding for?
Salary:Input the full, annual salary of the position you are requesting funding for. This amount does not include fringe or benefits.
% of time (FTE):What percentage of the position's time will be funded through this request? For example, if you have an existing position that will be spending half their time working on the project supported by this funding, you would input 50%. If you are requesting funding for a brand new position that will be completely supported by this funding, you would input 100%.
# of months requested:How many months will this position be supported by this funding? The maximum length of time this funding can support a position is 24 months (07/01/27 - 6/30/29).
Total Salary:Formula Cell - do not enter. Total Salary calculates "Annual Salary" x "% of time" x "# of months requested"
Narrative:Briefly explain what each position will be doing to support the project.
(2) Fringe BenefitsThis is your opportunity to incorporate fringe benefit costs associated with the positions. Fringe benefits may include health insurance, retirement costs, etc.
Position #:This field should correspond with Position # from Budget Category 1 (Salary).
Total Salary:Formula Cell - do not enter. Auto populated from cells in Budget Category 1 (Salary).
Base or %Use either Base or Percentage, depending on how your organization calculates fringe costs. The totals will auto populate.

Base is calculated by adding the specific dollar cost of each benefit (e.g., health insurance, retirement). If you are providing a base/actual amount, you will enter the total fringe per position in each line in the "Base" column of the Fringe section. You will then provide the breakdown of how the base was calculated, per the Fringe section instructions.

Percentage is calculated by applying a single percentage rate (e.g. 28%) to the salary amount requested for the personnel. If you are providing percentage based fringe costs, you will enter the percentage in the "%" cell for each position for which you have requested salary.

=Do not enter anything in this field.
Total Fringe:Formula Cell - do not enter. Total Fringe calculates "Total Salary" + "Base" x "%"
(3) EquipmentInput the total estimated cost of all anticipated equipment needs for the program in the dollar field. Equipment refers to items that are built to last for more than one year and cost less than $5,000 per item.
Narrative:Describe the specific items being requested, such as computers, printers, telephones, or other tangible personal property, and include cost estimates for each item. Provide clear calculations demonstrating how the totals were determined (for example: “Printers: $500 each × 2 = $1,000; Computers: 4 staff × $800 = $3,200”). Additionally, explain why each piece of equipment is necessary to support the proposed program, including how the equipment will enhance service delivery, program operations, or administrative capacity.
(4) Capital ExpensesInput the total estimated cost of all anticipated capital expenditure needs for the program in the dollar field.
Please outline and describe the type of Capital Expense: Capital Outlay (durable items costing $5,000 or more) or Capital Improvements (remodeling or construction between $5,000 and $250,000).

(5) Supplies Input the total estimated cost of all anticipated supplies needed to support the program. Supplies may include materials such as paper, pens, binders, folders, highlighters, computer disks, and meeting-related items. You do not need to list each item individually; only the total estimated cost is required.

(6) TravelEstimate total travel costs associated with the program. Trips should be categorized as in-state or out-of-state, and include total costs for multiple trips and/or multiple staff.
Narrative:Describe the purpose of your proposed trips and how they relate to project activities. Include local, in-state, and out-of-state travel and describe the staff, total per diem, total hotel, total airfare, and total registration fees in the estimated budget.
Cost Estimates:Include estimates for each of the subcategories (Total Per Diem, Total Hotel, Total Airfare, Total Registration Fees, Other). You can use the US General Service Administration website to estimate per diem rates: https://www.gsa.gov/travel/plan-book/per-diem-rates. Input total estimated miles driven in the Miles field to automatically calculate mileage cost).

(7) Other Input anticipated costs that don't fit into the other categories, such as food, gift cards for participants, equipment rental, printing, etc. List each item individually and provide a brief description, quantity, and cost per unit. Provide a brief justification within the Narrative space that describes how each item will support project activities or outcomes.

(8) ContractsList all contractors and sub awardees that will support the project
Contract Information:List all contractors or sub awardees on separate rows. Briefly describe the services they will provide or the activities they will complete. Input the total funding amount for each contract.

**Please note that, if awarded, OHA requires additional information for contractors and sub awardees. Grantees with Contract expenses will be required to submit a separate document that includes (1) scope of work, including tasks and deliverables; (2) time period of the contract; (3) person in your agency who will supervise or manage the contract; (4) name of the contractor, if known; and (5) what method will be used to select the contractor (e.g., bids, Request for Proposals, etc.).

Narrative: Describe how each contractor or sub-recipient will support project activities and/or project outcomes.

(9) Total Direct Costs Formula Cell - do not enter. Auto populated from Budget Categories (1) thru (7)

(10) Cost Allocation and Indirect Rates Input your organization's indirect rate as a percentage. Once the percentage has been entered, the dollar amount will auto populate based on "Total Direct Costs" x "Indirect Rate."

Allowable costsAllowable Costs are expenses that directly support the approved project activities. To be allowable, a cost must be necessary to carry out the funded work, reasonable in amount relative to the project’s scope, and allocable, meaning it provides a clear and measurable benefit to the Opioid Settlement Community Grant–funded activities. Allowable Costs must be supported by adequate documentation and must not duplicate, replace, or supplant existing funding sources.
Budget periodThe budget period for Opioid Settlement Community Grants is July 1, 2027 - June 30, 2029. Your budget should include all estimated program costs. Your total budget should fall within the award range for the funding tier for which you are applying (Tier 1: $25,000-$75,000; Tier 2: $100,000-$500,000).

You do not have to budget for the full 24-month grant period. You may complete your project activities at any time before June 30, 2029.

Capital expendituresCapital expenditures are major costs that help build, improve, or expand a long‑term project. These include two types of investments: Capital Outlay (durable items costing $5,000 or more) and Capital Improvements (remodeling or construction between $5,000 and $250,000). Under Oregon Administrative Rule, Chapter 309, Division 37, grant‑funded capital projects require a Declaration of Restrictive Covenants (DRC). A DRC is a recorded document that protects public investment and ensures the property continues to be used for the project described in the Grant Agreement throughout the required covenant period.OAR 309-013-0130; OAR Chapter 309, Division 37.
ContractorA contractor is an organization or individual that your program pays through a contract to provide specific, well‑defined services or deliverables, such as IT support, evaluation, or training. A full budget narrative will need to be completed by any subawardee or contractor that you choose to work with.
Direct costsDirect Costs are reasonable and necessary costs that are directly attributed to the approved project activities. These costs directly support the project and may include items such as staff time dedicated to project work, materials or supplies used exclusively for project activities, equipment essential to implementation, and travel necessary for carrying out project tasks.
EquipmentEquipment refers to items that are built to last for more than one year and cost less than $5,000 per item. These are durable, long‑term assets the program will use over time, such as computers, printers, or other sturdy pieces of technology or furniture.
Funding tierThe Opioid Settlement Community Grants has two funding tiers. Tier 1 is for projects ranging from $25,000-$75,000. Tier 2 is for projects ranging from $100,000-$500,000.
Indirect costsIndirect Costs are operating expenses that are not connected to a specific project, product, or task. These costs support an organization's general operations and business functions. Examples include rent, utilities, insurance, and legal or accounting fees. There is a 15% cap on indirect costs for Opioid Settlement Community Grants.
Other expensesOther Expenses are direct project costs that do not fall under standard categories (such as personnel, travel, equipment, capital expenditures, or supplies) but are necessary for carrying out the approved project activities. These expenses may include items such as communications and outreach materials, postage, printing, translations or interpretation services, event‑related costs, insurance premiums, or minor materials not classified as equipment. For “Other Expenses” to be allowable, they must be reasonable, necessary, and clearly connected to the project’s objectives and supported by adequate documentation.

If you are unsure about where an item may fit within your budget, include it within the "Other" category.

Sub awardeeA non-federal entity that receives a subaward from the Opioid Settlement Community Grants awardee and uses proposed funds to support, participate, or carry out activities for at least one of the proposed project activities. A full budget narrative will need to be completed by any subawardee or contractor that you choose to work with.
SuppliesSupplies are items that are used up during normal program activities and do not last long enough or cost enough to be considered equipment. These are everyday materials the program needs to operate, such as paper, pens, folders, binders, highlighters, and similar consumable items.
