About this file
This document is a proposer information and certifications form for the Oregon Health Authority's Opioid Settlement Community Grants program (RFGP 6312). The form serves as the initial eligibility screening and organizational documentation requirement for organizations seeking funding to address the opioid crisis in Oregon. Applicants must demonstrate that their organization is physically located and legally registered in Oregon, that their project will primarily serve Oregon residents, and that the proposed work aligns with at least one of five funding categories: Primary Prevention, Overdose Prevention and Risk Reduction, Treatment, Recovery, or Innovative Research. The grant agreement covers a two-year period from July 1, 2027, through June 30, 2029, with all funds required to be spent or fully reserved for specific costs by June 30, 2029. Projects may be completed before the end of the two-year term without penalty.
Eligibility requirements include organizational experience relevant to the proposed project, commitment letters from all named partner organizations, and a requirement that the funding be used for new projects or to expand or enhance existing work rather than replace or duplicate existing state, local, tribal, or private funding. State agencies and tribal governments are ineligible to apply directly, though other organizations may partner with these entities. Applicants must certify their financial capability, provide details of similar completed contracts or grants within the past five years, disclose any criminal or civil charges, contract terminations, lawsuits, judgments, bankruptcy filings, or financial distress, and confirm possession of all required licenses and certifications to conduct business in Oregon. The form requires detailed organizational information, authorized signatory designation, and fiscal sponsor information if applicable, with all required fields marked with an asterisk subject to verification by Oregon Health Authority staff.
