About this file

This is a Sample Grant Agreement template issued by the State of Oregon, acting through its Oregon Health Authority (OHA), for the Opioid Settlement Community Grant program (RFGP S-44300-0001737/6312). The agreement establishes the terms under which grant recipients will receive funding to address the ongoing impacts of the opioid crisis through one of five funding categories: Primary Prevention, Overdose Prevention and Risk Reduction, Treatment, Recovery, or Innovative Research. The grant agreement becomes effective upon execution of all required signatures and permits performance to commence on July 1, 2027, with reimbursement of eligible expenses following execution. The agreement term extends through June 30, 2029, unless extended or terminated earlier in accordance with its provisions. The specific project description, disbursement schedule, and special provisions are to be completed in Exhibit A upon outcome of the RFGP process, and the maximum grant amount payable to the recipient is to be determined (TBD).

Grant funds are disbursed via Electronic Funds Transfer (EFT) within forty-five days of OHA's acceptance of the recipient's invoice, provided the grant amount does not exceed the not-to-exceed maximum specified in the agreement. The recipient must return any unexpended funds within fourteen calendar days of agreement termination or expiration. Recipients are required to comply with comprehensive language access and communication support requirements, including qualified interpreters, translated materials, and alternative formats for individuals with disabilities, with costs included in the grant budget. Recipients must maintain specified insurance coverage as detailed in Exhibit C, with types and limits determined by OHA upon issuance of the Notice of Intent to Award based on risk assessment of the proposed project. All recipients must comply with federal and state laws, maintain detailed financial records for six years following final disbursement, and certify compliance with various requirements including the Oregon False Claims Act, non-discrimination provisions, and verification that principals are not on federal exclusion lists.
