About this file
This is a Disqualification Disclosure form required by the Rose Bowl Operating Company (RBOC) in Pasadena, California, pursuant to California Public Contract Code Section 10162. The form requires prospective bidders to disclose under penalty of perjury whether the bidder, any officer, or any employee with a proprietary interest has ever been disqualified, removed, or otherwise prevented from bidding on or completing a federal, state, or local government project due to violations of law or safety regulations. Bidders must indicate yes or no and provide explanatory documentation if applicable.
This disclosure requirement is part of the procurement process for the Construction Manager at-Risk (CMAR) selection for the Rose Bowl Stadium Plumbing Infrastructure Enhanced Maintenance Project. The RBOC is soliciting qualifications, technical proposals, and price proposals from contractors to provide preconstruction and design-assist services, including a funded subsurface field investigation program, and potentially construction services for replacement and rehabilitation of the stadium's water, sewer, and gas distribution systems. The project requires CMAR firms with demonstrated experience in wet-utility work, open-cut main replacement, trenchless sewer rehabilitation, live-venue phasing, subsurface risk management, financial strength, strong safety records, and commitment to local Pasadena-based subcontractors and workforce.
