INSTRUCTIONS:

Answer the following 4 questions and put your responses in the form above:

(1) The government plans to publicly release the registration information for industry day attendees, including the vendor's chosen super power. The government also plans to post this registration information on Federal Business Opportunities (FBO.gov) and provide it to attendees on the day of the event. Do you consent to the public release of your registration information? (Yes or No)

(2) The government plans to host short one-on-one sessions between the vendors and the government technical team on the day of the event in order to allow vendors to ask very a few very important and focused questions to help facilitate their decision making regarding this contract opportunity. 1-on-1 sessions are expected to last 3-5 minutes each, they will occur in the afternoon portion of the event, and interested vendors will be assigned an exact time and notified of their exact session time upon arrival on the day of the event. Do you wish to participate in a one-on-one session with the government technical team? (Yes or No)

(3) What is your company's superpower? The superpower (a strength, specialization or niche of a company) is intended to facilitate teaming conversations among vendors. That superpower will be stated on your name badge at the event and also will be listed on the registered vendor list that the government plans to publicly post. (Pick 1 superpower)

(4) At the industry day event on June 29, the government plans to host a vendor-to-vendor "speed dating" or networking session in the afternoon in order to facilitate teaming discussions between vendors.
