About this file

This is a Construction Bid Submission Review Form created by the State of West Virginia Department of Environmental Protection to guide potential bidders on submission requirements and common disqualification issues for construction projects. The form serves as an informational resource outlining the bidding process standards rather than project-specific details. Potential bidders must review all associated request for quotation documents, addenda, and instructions in their entirety to understand specific project requirements. The form emphasizes that failure to comply with stated requirements may result in bid disqualification, and the list provided does not represent an exhaustive compilation of all possible bidding requirements or grounds for rejection.

Immediate bid disqualification results from critical failures including non-attendance at mandatory pre-bid meetings, failure to sign required signature pages, missing West Virginia contractor's license information, lack of signed drug-free workplace affidavits, invalid or missing bid bonds, non-compliance with mandatory RFQ requirements, failure to acknowledge receipt of addenda when required, late bid submissions, federal or state debarment status, outstanding environmental enforcement actions, and unresolved DEP vendor complaints. Additional disqualification factors that must be resolved before contract award include uncontested debt to the State exceeding $1,000, Workers' Compensation or Unemployment Compensation delinquency, and failure to obtain required bonds and insurance coverage. The form does not address specific pricing, project scope, contract terms, funding sources, or set-aside provisions.
