About this file

This document is a Contractor Responsibility Information form from the City of Tucson, designed for the Apparent Low Bidder to complete and submit within five full business days after bid opening. The form requires the contractor to provide comprehensive details about their company's qualifications, past performance, and ability to execute the project, including certification of their legal and financial standing. The document seeks to verify the contractor's responsibility by examining factors such as debarment status, prior contract performance, legal history, financial stability, and current workload.

The form mandates that contractors disclose critical information including any history of liquidated damages, contract terminations, legal claims, bond issues, and potential legal or bankruptcy proceedings. Contractors must also provide their current Experience Modification (EMod) Rating and detail three representative projects similar to the current project's scope, including project names, locations, owner references, work descriptions, subcontractor information, total project values, and construction schedules. The City of Tucson reserves the right to contact references and may request additional documentation to support the contractor's claimed qualifications. Failure to provide sufficient information or making false statements could result in the contractor being found non-responsible and potentially disqualified from the bidding process.
