Instructions: Fill in the boxes for “Proposed Overhead”, “Proposed Profit”, “Proposed Bond”, and
“Proposed Express Menu” for each Zone for which you are submitting your proposal. The price form must be filled in completely for each Zone for which you are proposing.
The proposed rates, once accepted and incorporated into a contract, will be the maximum that can be applied for any task order under the contract.
“Proposed Overhead”, “Proposed Profit”, and “Proposed Bond” are to be applied to the awards of task orders, as well as to any modifications to task orders.
By submission of this Contract Price Form and SF 1442, the signer of these documents agrees to be bound by these rates for the life of any resulting IDIQ contracts and task orders thereunder, and agrees that these prices are available for award for at least 90 days from the due date of proposals. The signer must also be authorized to act on behalf of the Offeror.
