About this file

This is a Consent to Search Form issued by the Colorado Department of Corrections (CDOC) dated January 1, 2014. The form serves as a visitor authorization and registration document that establishes conditions for entry into CDOC facilities and offices. The document outlines 17 categories of contraband items prohibited from introduction into any CDOC facility or location where inmates in state custody are present, including dangerous instruments, controlled substances, currency, electronic communication devices, tobacco products, and other items that pose security threats. Visitors must acknowledge they have read, understood, and will comply with these regulations by signing the declaration of consent and waiver section. The form includes sections for recording guest data, vehicle information, and facility destination details, with language provided in both English and Spanish.

The form establishes that entry into CDOC property constitutes consent to search of the visitor's person, personal property, minor children accompanying them, and any vehicles brought onto facility grounds. Violations of the contraband provisions or submission of false information on the form may result in the visitor being barred from or denied access to the Colorado Department of Corrections State Penitentiary or any of its facilities, and may subject the violator to criminal prosecution under specified Colorado Revised Statutes (CRS 12-22-203, 12-22-303, 12-46-103, 12-47-103, 18-7-101, and 18-18-102 through 426). The form does not involve pricing, contract terms, or procurement activities, but rather functions as an administrative control document for facility security and visitor management.
