The form mandates that all proposers disclose any associations between their business and elected or appointed City of Tampa officials, City employees, or their immediate family members and close personal relations. Proposers must indicate whether such associations exist and, if so, provide the names and positions of the individuals involved. The RFQ was released on October 27, 2025, with proposals due by November 17, 2025, at 4:30 pm. A non-mandatory pre-proposal meeting is scheduled for November 4, 2025. The City intends to pre-qualify a minimum of three respondents to provide ongoing and as-needed plan review and inspection services for residential and commercial structures within City limits, covering building, electrical, mechanical, and plumbing trade categories in accordance with Florida Statutes 468, 471, and 481. The contract term is anticipated to be one year with options for three additional one-year renewals.

The disclosure form defines immediate family as spouse, parents, and children, while close personal relations include dating, cohabitation, or intimate sexual relationships. A controlling financial interest is defined as direct or indirect ownership of ten percent or more of outstanding capital stock or equivalent interest in a business entity sufficient to control operations. Proposers must complete the form by printing their firm name, individual name, title, and date, along with a signature certifying the accuracy of their disclosure. The City's evaluation criteria include cost to the city (10 points), firm experience and project team qualifications (30 points), references (15 points), understanding of scope of work (25 points), criminal history screening practices (5 points), and Small Local Business Enterprise participation (15 points). Compensation will be structured as an all-inclusive unit price per hour model with no additional amounts permitted beyond the established rate.
