About this file

This document is a Proposal Signature Form for the City of Tampa, specifically a signature page for a Request for Qualifications (RFQ) package. The form requires a proposer to sign and confirm compliance with the RFQ package requirements, including the Communication Policy and City of Tampa Ethics Code contained in the "General Conditions" section. The form provides specific instructions for corporate signatories, noting that when a corporation is proposing, the president, vice president, or another duly authorized representative must sign with their full name, title, and corporate seal, and proposals signed by non-officers must be accompanied by evidence of authority.

While the document does not provide details about the specific project, services, or contract terms, it serves as a critical legal document ensuring that the proposing entity understands and agrees to the terms of the RFQ and commits to ethical conduct in their proposal submission. The signature form is a standard procedural document designed to validate the proposer's intent, authority, and compliance with municipal procurement guidelines.
