About this file

This Task Assignment Change Order Form is issued by the Florida Department of Environmental Protection (FDEP) and involves JGP Structural Group, Inc., a structural engineering firm based in Tallahassee, Florida. The change order modifies an existing contract (CN217, Task Assignment #29) for dock repair services at Cayo Costa State Park. The document is signed by authorized representatives from FDEP, including the Budget Representative, Project Manager, and Contract Manager, as well as by Jeffrey Parzych, PE, representing JGP Structural Group, Inc.

The change order authorizes additional engineering and permitting services required due to site damage caused by Hurricane Ian. The modification adds $14,880.00 in fixed fee costs to the existing task assignment, bringing the total contract value to $48,751.10 ($658.60 reimbursable plus $48,092.50 fixed fee). The additional services include three subtasks: asbestos survey and lead paint testing ($3,940.00), site visit and revised final design documents ($7,270.00), and permitting services to revise and resubmit permit applications to the South Florida Water Management District and Army Corps of Engineers ($3,670.00). The change order also extends the task execution period by 365 calendar days, with the new task end date set for June 28, 2024. Payment is structured as fixed fee compensation upon FDEP's approval of deliverables for each subtask, with all work to be completed within specified timeframes ranging from two to four weeks from receipt of task approval.

• JGP will perform site visit(s) to verify existing conditions. The areas of the removed piles (performed by the demolition contractor), the areas of the remaining piles to allow temporary use of the staff dock and the impact of uprooted trees and damage of the existing concrete rip-rap will be documented. This information will be used to modify the previously provided "final" drawings and the permit applications to depict the current conditions regarding the fixed and floating docks.

Portions of the docks were damaged during Hurricane Ian and partial sections of dock were removed by the demolition contractor. Both the demolition drawings and the drawings depicting the previous existing conditions will be modified. Since many of the piles were located adjacent to concrete rip rap information regarding the damaged or disturbed areas including areas where new rip rap will be required will be added to the drawings to address these issues.

• When determining areas of work and repair, due to the observation techniques used, only visible conditions can be noted. Additionally, it must be understood that the drawings do not attempt to identify hidden conditions and defects. Additional work could be required beyond what is noted on the drawings. The extents of some conditions may not be known until construction.

• It is assumed that removed piles were completely removed and no broken sections are present.

• Engineering design, analysis and code verification of elements outside of the scope of work described above is not included.

• The work will be performed with the degree of skill and the standard of care common to the profession. No warranty is expressed or implied by this agreement.

• Trimming of vegetation, if needed, is to be provided by Park personnel.

• The Final design documents shall be formatted to the BOC 24"x36" standard and include the standard BOC border. Each individual sheet shall be clearly labeled and dated.

• The Final technical specifications shall be placed on the drawings or formatted to the BOC 8-

1 /2"x11" standard. The specification sections will be clearly labeled and dated.

• The Final documents shall be submitted to the BOC project manager via electronic file transfer in

PDF and AutoCad format. A revised Opinion of Probable Construction Cost (OPC) will be provided.

Deliverable: The "Revised" Final Design Documents and the opinion of probable cost (0PC) will be provided in electronic format to BOC for review. These documents will be provided within four (4) weeks after receipt of the approved TA.

• JGP will provide the "Revised" Final signed and sealed Design Documents in electronic format to

BDC. Final CAD files will also be provided.

• The 0PC will be provided in pdf format.

Performance Measure: Deliverables will be reviewed by the Department to confirm they are complete, accurate, and timely.
