About this file

This is a Contract Data Requirements List (CDRL) template on DD Form 1423-1, which specifies data deliverables required from a contractor during contract performance. The form is used to document administrative and management data that must be provided by the contractor to the government.

The specific data item detailed on this form is an Accident/Incident Report (Data Item No. A005) for the Bldg 228 Access Control System Maintenance contract. The report must comply with DI-SAFT-81563 and DAFI 91-204 standards. The contractor is required to notify the Contracting Officer, Contracting Officer's Representative, or other Military Flight Team members within eight hours of any damage to government property exceeding $600,000 in value, or within two workdays for damage between $600,000 and $1,000,000. No more than one late mishap notification per contract year is permitted. The report must be submitted within 48 hours of the mishap and must include contract number, personnel names and titles, date and location of incident, narrative description, cause, estimated repair costs, equipment nomenclature, corrective actions, and other pertinent information. Distribution is limited to 78 ABW/SCO and Robins AFB, GA. The data item applies across multiple contract line items (Exhibits A through E) covering the base year and four option years. The form was prepared by Benjamin L. Jowers on 25 September 2025. The instructions provided on the back of the form outline proper completion procedures for both government personnel and contractors, including pricing group classifications and estimated cost calculations for data item development.
