About this file
This document is a Bid Form for a municipal construction project with the City, detailing contract requirements and bidder representations. The project has a total completion time of 180 calendar days for the base bid, with an additional 60 calendar days available for an alternate bid. The successful bidder will be required to sign the Contract Agreement within 10 days of the City's Notice of Award, and the bid will remain open for 30 days after the bid opening date. The bidder must acknowledge receipt of all contract documents, examine the work site and legal requirements, and intends to perform a substantial portion of the work themselves.
The bid form requires the bidder to accept liquidated damages provisions, submit contract bonds, provide a certificate of insurance, and complete all work specified in the contract documents. The bidder must also indicate their business structure (individual, partnership, or corporation) and provide contact information including firm name, signature, title, address, and telephone number. The document leaves space for the bidder to specify the percentage of work they will perform and provides a framework for submitting the bid with all necessary documentation and representations.
