About this file

This is a Past Performance Questionnaire form for the Landing Gear Collaborative Supply Chain Integration II (LG-CSCI2) program solicitation SPRHA1-26-R-1003. The questionnaire is designed to collect performance feedback from government customers and relevant stakeholders regarding contractor performance on related landing gear and supply chain management efforts.

The form requests identification information including contractor name, CAGE code, contract number, period of performance, and contract dollar amounts. Respondents rate contractor performance across three primary technical factors: Engineering (landing gear expertise, quality deficiency resolution, and manufacturer oversight), Program Management (supply chain management, subcontractor performance, subcontract administration, customer responsiveness, and risk management), and Order Fulfillment (on-time delivery, logistics support, order processing, cost management, and financial controls). Each assessment element uses a four-level rating scale: Acceptable (A), Unacceptable (U), Not Applicable (NA), or Unable to Assess (UTA). The questionnaire also includes general questions regarding organizational structure effectiveness and other similar contracted efforts. Completed questionnaires should be submitted to Nathan Flinders at Nathan.flinders@dla.mil or (385) 591-1416, with William Brown identified as the Government Contracting Officer at (385) 519-8039. This is a template document with no specific response deadline provided in the questionnaire itself.
