About this file
This is an instruction sheet and template for submitting past performance information as part of a proposal response to solicitation SPRHA1-26-R-1003 for the Landing Gear Collaborative Supply Chain Integration 2 (LG-CSCI2) program.
Offerors must complete a separate Past Performance Information Sheet for each contract or program being submitted. The template requires completion of 13 sections including: offeror company information and CAGE code; program title; contract specifics (contracting agency, contract number, role as prime or subcontractor, contract type, original and current contract values, and period of performance); primary customer points of contact for the program manager, contracting officer, and administrative contact; unique considerations relevant to the work; key personnel proposed for the instant acquisition; brief program description; and relevancy to technical sub-factors. The relevancy section specifically requires offerors to demonstrate how their past performance on the submitted contract applies to three technical sub-factors: Engineering, Program Management, and Order Fulfillment, or mark as "N/A" if the team member will not perform in that area. Submissions are limited in number and length per Section L, Instruction to Offerors paragraph 4.4, with text boxes that expand as needed up to specified page limits. The sheet itself should not be reformatted in color, font, or structure, and should be submitted without the instruction page. Past performance relevancy will be evaluated based on similarity of prior work to the LG-CSCI2 requirements and the specific role the submitting team member will play on the acquisition.
