About this file
This document is a Past Performance Questionnaire template used to evaluate a contractor's performance across multiple assessment elements. The questionnaire provides a standardized rating system with five performance levels: Very Good (VG), Satisfactory (S), Marginal (M), Unacceptable (U), and Not Applicable (NA). It covers seven key performance areas including cost/price control, schedule adherence, overall performance, quality, business relations, personnel qualifications, and an overall performance rating. The form requires detailed supporting narratives for each rating, allows identification of contractor strengths and weaknesses, and includes a section asking whether the evaluator would choose to award to the contractor again. The final section includes a certification statement where the evaluator signs and dates the document, attesting to the accuracy of the ratings and comments.
