About this file

Attachment N is a certification form required by the Florida Department of Management Services for vendors responding to the Medium and Heavy Trucks Invitation to Bid (ITB). The form must be completed and signed by a member of the vendor's senior management, as defined under Florida Statutes section 287.1346, before submitting a response to the solicitation or entering into a contract. Senior management is defined to include chief executive officers, assistant chief executive officers, vice presidents, chief financial officers, and chief personnel officers, or any employee performing similar functions. The vendor certifies that the commodities being offered to the Department have not been produced, in whole or in part, by forced labor, and must provide the entity name, senior management member's printed name, title, signature, and date of certification on the form.

The Medium and Heavy Trucks ITB is administered by Alyssa Wallace, Purchasing Analyst with the Florida Department of Management Services, Division of State Purchasing. The State intends to establish a statewide term contract with the possibility of making multiple awards; however, the Department reserves the right to award to one or multiple bidders statewide or by region, or to make no award at its discretion. Interested vendors must submit responses through MyFloridaMarketPlace Sourcing Event via the SAP Business Network and are responsible for monitoring the Vendor Information Portal (VIP) throughout the procurement process for updates and decisions. All correspondence to the procurement officer must include the solicitation number in the subject line. Bidders should direct inquiries to Alyssa.Wallace@dms.fl.gov or (850) 410-2426.
