About this file

This is a Vendor Certification Form (PUR 7801) issued by the Florida Department of Management Services for the Medium and Heavy Trucks Invitation to Bid (ITB). The form serves as a compliance document that vendors must complete and submit as part of their bid response to establish a State Term Contract for Medium and Heavy Trucks. The Department intends to make multiple statewide awards; however, it reserves the right to award to one or multiple bidders statewide or by region, or to make no award at its discretion. Vendors must submit their responses through the MyFloridaMarketPlace Sourcing Event via the SAP Business Network. The procurement officer is Alyssa Wallace, Purchasing Analyst, located at the Florida Department of Management Services, 4050 Esplanade Way, Suite 360.6Y, Tallahassee, FL 32399-0950, with contact information of (850) 410-2426 and Alyssa.Wallace@dms.fl.gov. All solicitation documents and addenda are posted on the Vendor Information Portal (VIP), which vendors must monitor throughout the procurement process.

The certification form requires vendors to attest to compliance with multiple Florida Statutes and regulations, including certifications regarding non-participation in boycotts of Israel, exclusion from scrutinized company lists related to Sudan and Iran activities, absence from suspended or convicted vendor lists, E-Verify system registration and use, compliance with disclosure requirements, non-use of coercion for labor, and non-placement on forced labor lists. For contracts valued at $1 million or more, vendors must additionally certify non-operation in Cuba or Syria and exclusion from scrutinized company lists. If the contract grants access to personal identifying information, vendors must complete Form PUR 1355 (Foreign Country of Concern Attestation Form). Common carriers must complete Form PUR 1808 (Common Carrier or Contracted Carrier Attestation Form). For commodity contracts, vendors must provide written certification from senior management that commodities have not been produced by forced labor. All certifications must be signed by an authorized representative and include vendor information, Federal Employer Identification Number (FEIN), signatory name, signature, date, typed or printed name, and title.
