Past Performance Questionnaire
Background: This questionnaire will be used to provide information to support the Missile Defense Agency Warfighter Operational Integration (WOI) services procurement, RFP HQ0858-20-R-0011
Message to the Assessor: Your feedback is requested by the Missile Defense Agency (MDA) to assist with establishing the performance history for the Contractor identified below. To expedite receipt of the requested information, MDA requests that you e-mail the completed questionnaire(s) to: mara.strobel@mda.mil AND TN-Warfighter@mda.mil. Your support and participation in this matter is essential and greatly appreciated!
Instructions for Offeror/Subcontractor and Assessor:
· Sections I and II to be completed by the Offeror/ Subcontractor and provided to the Assessor (the Government personnel that had experience with the Offeror/Subcontractor on a previous relevant contract). The assessor, in turn, to verify Sections I through II, complete the questionnaire, and submit to the Contracting Office.
· Section II to be validated by the assessor. If any information is incorrect herein, please annotate and provide the correct information accordingly.
· Sections III through VI to be completed in their entirety by the assessor.

Supporting operational watch standing functions to include dealing with senior leaders to report health and status of a major weapon system.
Conducting or facilitating Asset Management procedures to integrate and synchronize geographically dispersed assets of a major weapon system.
Maintaining the software and hardware configuration of a major weapon system in an approved operational baseline.
Building/maintaining professional relationships to support operational-level interface with combatant commanders and their staffs to increase and improve Warfighter participation in developing future missile defense capabilities. (Expert knowledge on the organization, management, leadership and duties of the U.S. Army, U.S. Air Force, U.S. Navy USSF, and Joint Staff and their ability to interact with CCMDs, JFCC-IMD and MDA on missile defense issues.)
Supporting development of Warfighter Involvement Products, such as the Modification and Fielding Request List, Prioritized Capabilities List, and Achievable Capabilities List that influence a major weapon system.
Supporting Sea-Based Weapon System programmatic efforts including operational integration; real-world events; technical integration, deliberate planning and training.
Supporting Ground-Based Weapon System programmatic efforts including integration and scheduling; development and fielding; sustainment engineering and operational support.
Supporting Sensor-Based programmatic efforts including operational integration; real-world events; sensor watch operations, and technical integration and deliberate planning.
