About this file
This is a Past Performance Questionnaire (PPQ) template for the FDA's Contractor-Hosted Fully Integrated Records Facility (FIRF) IDIQ Recompete solicitation (RFP 75F40126R00009). The questionnaire is to be completed by past performance references and submitted to FDA Contract Specialist Melinda Kim and Contracting Officer Nick Bisher by the proposal due date. The form requests references to provide objective ratings across four performance categories: Business Relations, Quality of Service, Cost Control, and Timeliness of Performance, with ratings ranging from 1 (Poor) to 4 (Excellent), or N/A if insufficient knowledge exists. References are asked to identify their role (Contracting Officer, Contract Specialist, Project Officer, or Other), provide contract details including value, performance period, contract type, and percentage of work performed by subcontractors, and include general descriptions of products and services delivered.
The questionnaire also requests subjective assessments through yes/no questions regarding contractor commitment to customer satisfaction, recommendation for award, likelihood of rehiring, and value received relative to cost, with space for detailed explanations and additional commentary supporting all ratings. Rating guidelines are provided to standardize evaluations across cost control, timeliness, quality, and business relations categories. This template serves as a standardized evaluation instrument to assess contractor performance history as part of the competitive evaluation process for the FIRF IDIQ contract, which will provide offsite document control services including receiving, processing, inventorying, scanning, cataloguing, storage, and disposal of documents and records materials for the FDA.
