About this file

The document is a Recorded Program Eligibility Questionnaire from the Department of Early Learning and Care (DELC) in Oregon, specifically from the Child Care Licensing Division. The form is designed to help determine whether a school-age program meets the requirements for becoming a Recorded Program. Programs must answer five key eligibility questions related to the nature of their activities, such as whether they are primarily a group athletic/social activity, focus on a single enrichment activity, are operated by a government entity, have participants 14 and older, or provide care while parents are on-site.

Programs that answer "yes" to any of the eligibility questions do not need to be recorded with DELC. Those unsure or answering "no" must complete the form with details about their program, including program name, contact information, county, children's ages supported, hours of operation, and whether they operate as a parent cooperative. After submission to CCLD.CustomerService@delc.oregon.gov, a Licensing Specialist will review the information and either provide guidance on becoming a Recorded Program or confirm that registration is not required. The form includes an attestation section for the program contact to verify the accuracy of the information provided.
