About this file

This is a Sample Customer-Specific Scope of Work (SOW) template document for Janitorial Services under Florida's State Term Contract No. 76111500-27-STC, issued by the Florida Department of Management Services. The document serves as a template to assist customers in defining specific janitorial service requirements and supplements the master contract's SOW without modifying its core terms. Customers may negotiate additional terms and conditions that supplement the STC, provided they do not conflict with the established contract terms, with conflicts resolved in favor of the customer. The template addresses key service components including facilities and locations, service and space types, total square footage, detailed janitorial tasks, work hours, and required supplies. Contractor staffing must be adequate to maintain high cleanliness standards, with a designated on-site supervisor available during business hours, and any subcontractors require pre-approval from both the Department and the customer. The contract requires a pre-service conference at least ten days before the start date to review work plans, submit background checks, and provide cleaning supply lists and personnel contact information.

The document emphasizes comprehensive safety and compliance requirements, including OSHA standards adherence and prior written approval of all chemical products with Safety Data Sheets (SDS), with particular restrictions on products with pH below 3 or above 11, or those rated as flammable or toxic. Contractors must obtain all necessary hazardous materials licenses and certifications required by local jurisdictions prior to contract execution. Quality control mechanisms include daily Shift Cleaning Reports, monthly summary reports with Quality Evaluation Forms submitted with invoices, and a performance-based penalty structure. Financial consequences for non-performance include $100 per 24-hour period for untimely customer inquiry responses, $500 per discovery for unauthorized chemical use or storage, $250 per day for unresolved quality deficiencies, $100 per day for insufficient cleaning product stocks, and $1,000 per unauthorized subcontractor discovery. Contractors must maintain a minimum two-week inventory of all cleaning products and supplies, and adjustments to invoices result from quality evaluation ratings, with "Below Standards" ratings triggering a 3 percent reduction per performance deficiency.
