About this file
This is a monthly reporting template for Breastfeeding Peer Counselors working with the South Dakota Department of Health WIC Program. The template requires detailed tracking of caseload statistics, including pregnant and breastfeeding women served across Central and West regions, specifically in Butte County (Belle Fourche and Newell locations) and Lawrence County (Deadwood and Spearfish locations). The report captures caseload changes, including start count, new referrals, transfers, drops, and end count, as well as tracking various types of participant contacts through Zendesk (texts/calls/emails), face-to-face consultations, Zoom consultations, mailed information, and announcements.
The template includes sections for reporting graduating participants (those with children reaching 1 year), breastfeeding classes led or assisted, support groups, outreach activities, impact on area/participants, trainings attended, and miscellaneous items. The form emphasizes proper tracking of all contact types and requires specific breakdown of announcements sent to prenatal and breastfeeding participants, with these numbers being included in total contact counts. The example provided shows a caseload starting at 151, gaining 17 new referrals, ending at 168, and projecting 159 for the next month's start after 9 drops.
