About this file
This is a monthly maintenance checklist for headstone, marker, and niche cover inspection at the Los Angeles National Cemetery. The checklist is designed to be completed by inspectors to assess the condition and maintenance status of cemetery grounds under the grounds maintenance services contract managed by the Department of Veterans Affairs National Cemetery Administration.
The checklist covers four primary inspection categories aligned with OAI (Oversight and Accountability Initiative) measures: Cleaning (OAI 4.1), which verifies that headstones, markers, and niche covers are free of debris and objectionable accumulations; Height and Alignment (OAI 4.2), which confirms proper positioning of upright headstones, flat markers, and niche covers; Condition assessments including inscription visibility and legibility (OAI 4.4), identification and action taken on damaged or defaced items (OAI 4.5), and proper disposal of unusable markers in a respectful manner (OAI 4.6). For each measure, the inspector documents findings as OK, Needs Attention (with reference number), or N/A, and includes comments. The inspector must sign and provide their title upon completion of the monthly inspection.
