About this file
This document is a U.S. Coast Guard meal sign-in sheet template (CG Form WS-4901, Rev. 07/23) used at Training Center Cape May to record meal transactions and track reimbursement by patron category. The form is designed for three daily meals: breakfast, lunch, and dinner, with separate pages for each meal period.
The sheet captures patron information including name, unit, and branch or organization, and categorizes meals into two rate structures: Standard Meal Rate and Full Meal Rate. Standard Meal Rate categories include active-duty personnel (AD), Coast Guard reservists on orders (RESO), Coast Guard Federal civilian employees (CIVCG), dependents of E-1 through E-4 active-duty members (DE1E4), and non-profit youth group members (NPYG). Full Meal Rate categories include non-Federal civilians (CIV NONFED), reservists not on orders (RESNO), auxiliarists not on orders (AUXNO), non-Coast Guard Federal civilian employees (CIV OTHFED), dependents of E-5 and above active-duty members (DE5ABV), retirees (RET), contractors (CONT), and others authorized by commanding officer (OTH). The form also designates columns for Subsistence-in-Kind (SIK) meals provided to cadets and recruits. Each page includes fields for cashier identification, received-by signature, meal rates, total meals sold, and cash and credit totals, with space for multiple patron entries and page numbering. Individual Credit Accounts (ICA) are not authorized at the Training Center Cape May Dining Facility.
