About this file

This is a Past Performance Questionnaire and Contractor Evaluation Form issued by the U.S. Coast Guard Operational Logistics Command (LOGCOM) for solicitation 70Z08426RDL950004 regarding Military Food Service Contracting at the U.S. Coast Guard Training Center in Cape May, New Jersey.

The form serves as a reference investigation tool to assess potential contractors' past performance on previous contracts. It is divided into two sections: the offeror section requiring the contractor to provide company information, contract details, and work descriptions, and the respondent section completed by reference contacts evaluating the contractor's performance. Responses should be submitted to Janine Marshall at janine.a.marshall@uscg.mil. The form requests detailed contract information including contract number, dollar values, periods of performance, and contract type (FFP, CR, T&M, IDIQ, SCA, Davis Bacon, Sole Source, or Subcontracting). The evaluation section uses a six-tier performance rating scale (E—Exceptional, G—Very Good, S—Satisfactory, M—Marginal, U—Unsatisfactory, N—Neutral) and assesses contractors across multiple performance categories: Quality (quality control plans, deficiency correction, personnel qualification), Schedule (progress schedules, project timelines, warranty defects, subcontractor payments), Cost Control (cost proposals, material management), Management (supervision, problem identification and resolution, responsiveness to changes, key personnel management), and Regulatory Compliance (environmental, labor, safety regulations). Narrative explanations are required for any ratings of E, G, M, or U, and the form includes sections for overall performance rating and inquiry regarding cure or show cause notices.
