About this file
This is a Responsibility Inquiry form for ITB S-47100-00013511, the Canyon City Janitorial contract administered by an unnamed state agency in Oregon. The form serves as a pre-award assessment tool to evaluate bidder qualifications and financial stability prior to contract execution. Bidders must respond to eight primary inquiry areas addressing resource availability, contract performance history, criminal and civil charges, contract termination history, financial condition, bankruptcy filings, licensing and business authorization, and possession of a current Pay Equity Certificate issued by the Department of Administrative Services. The form requires bidders to document the number of similar contracts completed within the last five years that were completed on time and within budget without claims, explain any contracts that did not meet performance standards, and provide detailed responses regarding any adverse legal or financial events within specified timeframes.
The Responsibility Inquiry form does not establish pricing terms, set-asides, funding amounts, or contract timelines; rather, it functions as a qualification verification document that agencies may supplement with additional documentation requests including profit-and-loss statements, balance sheets, cash flow information, assets-to-liabilities ratios, bonding capacity assessments, credit information, and performance records under previous contracts. Failure to promptly provide requested information or demonstrate responsibility may result in a finding of non-responsibility and bid rejection. The form must be completed and signed by an authorized representative of the bidding firm, with the signatory certifying that all responses are complete, accurate, and not misleading to the best of their knowledge.
