Patient Information Card - Sexual Assault Evidence Collection Kit Attachment

Attachment 7 is a patient information card component for the Sexual Assault Evidence Collection Kit Master Agreement issued by the State of Maine, Department of Administrative and Financial Services, Office of State Procurement Services, on behalf of the Department of Public Safety. This document serves as a standardized form included within the fully assembled evidence collection kits that vendors must supply. The procurement seeks approximately 1,000 kits annually on an as-needed basis, with delivery to the Maine State Police Crime Lab in Augusta on a FOB Destination basis. Quotations are due by May 6, 2026, at 4:00 p.m. Eastern Standard Time through the State of Maine's AdvantageME Vendor Self Service system. The contract term runs from June 1, 2026, through May 31, 2027, with four optional extension periods extending through May 31, 2031. Delivery is expected no more than six weeks after artwork finalization, with kits packaged in numerical order in shipping containers.

The patient information card component must comply with Maine's forensic standards and law enforcement protocols, and vendors must submit physical samples of complete kits, including this card, for evaluation by the requesting department prior to bid closing. Pricing will be established based on vendor quotations and the State's evaluation of the lowest-priced qualified proposals. The State reserves the right to make a single award to the lowest-priced qualified bidder or multiple awards at its discretion. Eligible participants in the Master Agreement include state police, municipal police departments, hospitals and healthcare facilities, and sexual assault crisis centers throughout Maine. Quoted prices must remain firm for a minimum of one year from the initial contract period. The State of Maine is exempt from federal, state, and local taxes and will issue an exemption certificate at contract execution.
